A 75 YO diabetic woman had white eruptions on the tongue for 3 days

หญิงอายุ 75 ปี มีอาการเจ็บคอและมีตุ่มขาวๆที่ลิ้นมานาน 3 วัน อาการเจ็บไม่รุนแรง
ผู้ป่วยเป็นเบาหวานและความดันโลหิตสูงมานาน 10 ปี

ตรวจร่างกาย: Appears well, afebrile
Tongue: multiple well-defined, white papules, 2 to 3 mm in diameter with a central punctum on the surface of her tongue (รูป A).

รูป B เป็น shave biopsy ของลิ้น

คำถาม

1. ผู้ป่วยน่าจะเป็นโรคใดมากที่สุด
A. Candidiasis
B. Aphthous ulcer
C. Herpetic glossitis

2. จะ manage อย่างไร

ขอบคุณคุณหมอ Sagittareusinternist มากค่ะ ที่ตอบมาถูกต้องแล้วค่ะ

-Shave biopsy: histologic findings typical of herpetic infection, including perinuclear halo, margination of chromatin, and multinucleated cells (arrows).
-Culture of the lesions showed herpes simplex virus type 1 (HSV-1).

herpetic glossitis มีลักษณะที่ typical และแตกต่างจาก herpetic vesicle ที่ผิวหนัง คือ herpetic glossitis เป็น discrete areas of ulcerationและเป็นรูปกลม Oral HSV-1 reactivation จะเป็นกับ keratinized surfaces ของปาก ขณะที่ primary HSV-1 infection มักเป็นที่ mucosal surfaces ลักษณะที่ typical นี้ทำให้แยกได้จาก oral candidiasis and aphthous ulcers

จากการติดตามที่ 2 สัปดาห์ พบว่า รอยโรคนี้หายไปเองโดยผู้ป่วยไม่ได้รับยาใดๆ
