เด็กชายอายุ 6 ปี มีไข้มา 3 เดือนและมีตาขวามัวลง




   เด็กชายอายุ 6 ปี มีไข้ที่ยังไม่ทราบสาเหตุมานาน 3 เดือนและมีตาขวามัวลง Ophthalmoscopy พบรอยโรคที่ peripheral retina ของตาขวา (รูป 1) และมี cells ใน vitreous.

Labs: CBC: WBC 8 500/µL with 17% eosinophilia.

LFT: AST 65 IU/L; ALT 115 IU/L; total bill 0.77 mg/dL

1. การวินิจฉัยน่าจะเป็นโรคใด

2. จะ manage อย่างไร

   Ophthalmoscopy : White masses were present in the superior peripheral retina. The track of larvae revealed in the fundus examination (รูป 1).
-Stool exam: eggs and larvae of parasites were negative.
Antibodies titers ใน serum สำหรับ ascariasis, trichostrongyliasis, hydatidosis, toxoplasmosis และ leishmaniasis ได้ผลลบ. ELISA using T. canis ES antigen ได้ผล low antibodies titer ใน serum ของผู้ป่วย

   การตรวจหา antibodies titer ต่อ T. cati ด้วย third-stage larvae ES antigenใน serum และ vitreous fluid ได้ผล strongly positive (ELISA). ดังนั้น ocular toxocariasis ในผู้ป่วยรายนี้จึงน่าจะเกิดจาก T. cati.

   ผู้ป่วยได้รับการรักษาด้วย albendazole (400 มก. วันละ 2 ครั้ง) และ oral prednisolone นาน 3 สัปดาห์. สองเดือนต่อมา ผู้ป่วยมีอาการดีขึ้นและการตรวจ eye ground ก็พบว่าดีขึ้น.

    Human toxocariasis เป็น helminth zoonosis ที่พบได้ทั่วโลก. ผู้ป่วยรายนี้เป็นชาวอิหร่าน. Epidemiological studies ใน Iran พบว่า Toxocara seroprevalence rate =25.6% ในเด็ก และ 5.2% ในคนที่มีความเสี่ยง. ผู้รายงานได้เคยศึกษาพบว่า Toxocara seroprevalence rate สำหรับ T. cati ในแมวจรจัดใน Iram สูงถึง 42.6%..

   Ocular abnormalities เป็นภาวะแทรกซ้อนที่พบบ่อยใน toxocariasis. Ocular toxocariasis มีลักษณะ typical คือ เป็น monocular disease ในเด็กเล็ก. ผู้ป่วยโรคนี้มาด้วย chronic unilateral uveitis และมี marked vitreous opacification อยู่เหนือ primary eosinophilic granuloma. ผู้ป่วยอาจมี exudative retinal detachment, posterior synechiae และ cyclitic membrane. Toxocara granuloma มีลักษณะเป็น white, dome-shape, and confined principally to the retina. Serological tests เข่น ELISA using Toxocara infective larval ES antigen เป็น gold standard สำหรับการวินิจฉัย ocular toxocariasis. Ocular involvement พบบ่อยใน toxocariasis ที่เกิดจาก T. canis, และพบที่เกิดจาก T. cati.ได้น้อย.
