   หญิงอายุ 76 ปีมาพบแพทย์เนื่องจากมีตุ่มนูนที่เจ็บหลายตุ่มที่เท้า (รูป 1). 2 สัปดาห์ก่อนหน้ามีรอยโรคนี้ ผู้ป่วยไป Venezuela. ที่นั่น ผู้ป่วยเดินบนทรายโดยไม่ใส่รองเท้า และรู้สึกเหมือนมีอะไรตำที่เท้า. รอยโรคเริ่มจากเป็นจุดสีดำหลายจุดและมีขนาดใหญ่ขึ้น. PE: round, creamy-white eruptions of 3 to 4 mm in diameter on her soles, toes, and periungual areas (รูป 1A).

   การวินิจฉัย: Tungiasis.
นี่เป็น typical case ของ tungiasis ซึ่งเป็น parasitic disease ที่เกิดจาก sand flea Tunga penetrans. Flea นี้ชอบสิ่งแวดล้อมที่อบอุ่นและแห้ง. Ttungiasis มีความชุกในชนบทของ Central และ South America, the Caribbean และ sub-Saharan Africa.    Life activities เช่น การหายใจผ่าน spiracles (รูป 2B, arrow) และการขับ feces (รูป 2C) ผ่านรูเปิดที่ผิวหนัง ก็สามารถมองเห็นได้. นอกจากนี้ ยังสามารถมองเห็น การบิดตัวของ midgut ที่เต็มไปด้วยเลือดของคน ใน eruptions บางอัน (รูป 2D ).

   การรักษาเริ่มแรกคือ complete surgical excision of the insect. ถ้าทำไม่ได้ ก็ใช้เข็มเขี่ย flea ออกมาทั้งตัว. (รูป 3). ต้องให้ tetanus prophylaxis ด้วย. ไม่จำเป็นต้องให้ systemic antibiotics ในผู้ป่วยส่วนใหญ่; อย่างไรก็ตาม อาจต้องพิจารณาให้ยา niridazole หรือ oral thiabendazole ในกรณีที่มีรอยโรคจำนวนมาก และเป็นหนอง.

Tungiasis ยังเป็นปัญหาทางสาธารณสุขที่สำคัญสำหรับชุมชนที่ยากจน.และแพทย์ต้องคำนึงถึงโรคนี้ซึ่งป้องกันได้.
