เด็กหญิงอายุ 9 ปี เจ็บคอและมีไข้.




   เด็กหญิงอายุ 9 ปีซึ่งปกติแข็งแรงดี มาพบแพทย์เนื่องจากเจ็บคอและมีไข้. PE: T 38.5°C. swollen and tender cervical lymph nodes, inflamed uvula, enlarged tonsils, and lesions on both the hard and soft palates (Panels A and B).

    รอยโรคนี้มีชื่อเรียกว่า “Doughnut” lesions

-Rapid antigen detection test for group A streptococcus ได้ผล positive.
-Throat swab culture ขึ้น group A beta-hemolytic streptococcus.

   “Doughnut” lesions เป็น erythematous papules with a pale center ซึ่งอาจปรากฏที่ทั้ง soft และ hard palates. การมี “Doughnut” lesions เป็น clinical sign ที่สัมพันธ์กับ group A streptococcus pharyngitis.

   ผู้ป่วยได้รับการรักษาด้วย oral amoxicillin นาน 10 วันและหายดีโดยไม่มีภาวะแทรกซ้อนใดๆ
