ชายอายุ 45 ปีมีอาการปวดท้อง RLQ และเบื่ออาหารมา 1 วัน




   ชายอายุ 45 ปีมีอาการปวดท้องและเบื่ออาหารมา 1 วัน. ผู้ป่วยไม่มีไข้. PE: tenderness in the right lower quadrant of the abdomen. Rebound was present; Blumberg’s sign was negative.
Labs: normal WBC with 82% neutrophils.
Abdominal ultrasonography : the results were inconclusive with regard to the presence or absence of appendiceal inflammation.
ผู้ป่วยได้รับการผ่าตัด appendectomy.
Appendix มีลักษณะปกติ, แต่ pathological analysis พบความผิดปกติดังในรูป A+B (hematoxylin and eosin)

   พบ Enterobius vermicularis ใน appendiceal lumen (รูป A และ B,) และมีการอักเสบเล็กน้อยของ mucosa.
E. vermicularis, หรือ pinworm, เป็น nematode ที่พบบ่อยที่สุดในเด็กเล็ก แต่ในผู้ใหญ่ก็พบได้. Gravid female pinworm เกาะกับ mucosa ใน cecum และเดินทางไปสู่ anus เพื่อวางไข่ที่ perianal skin. และทำให้เกิด perianal pruritus.

   Autoinfection และtransmission เกิดขึ้นได้จากการกินไข่ที่ปนเปื้อนที่มือ ของใช้ในห้องน้ำ ที่นอน อาหาร และแหล่งอื่นๆ. บอกได้ยากว่า การมี pinworm อยู่ใน appendix เป็นสาเหตุของอาการนำของผู้ป่วยหรือเป็น incidental finding. หลัง appendectomy ผู้ป่วยและครอบครัวได้รับการรักษาด้วย mebendazole. การติดตามที่ 6 เดือนหลังการผ่าตัด ผู้ป่วยไม่มีอาการใดๆ.

    Key diagnostic features ของ Enterobius คือมี prominent lateral alae ซึ่งทอดยาวตลอดความยาวของพยาธิ cross section ของพยาธิจึงเห็นเป็นหนามแหลมอยู่ 2 ข้างของตัวพยาธิ
