PE: epigastric tenderness and hepatomegaly.
Labs: WBC 14,000 cells/µL, Hb+platelet ปกติ. Coagulation profile: ปกติ

    Ultrasonography of the abdomen disclosed an area of altered echogenicity associated with breakdown of tissue in the left lobe of liver. This region contained a coiled, mobile, hypoechoic, tubular structure (Supplementary Video 1, available online).

    Liver abscess ที่เกิดตามหลัง biliary ascariasis เป็นภาวะแทรกซ้อนที่พบได้ยากของ Ascaris infestation ซึ่งอาจเกิดจาก adult worms หรือไข่ที่อยู่ใน biliary channels หรือ liver parenchyma, และในบางราย larval stages ของ Ascaris เข้าไปใน hepatic parenchyma.

   การรักษา สำหรับ multifocal abscesses ที่เกิดจาก ascariasis การรักษาที่ recommend คือการผ่าตัดแล้วตามด้วยการกินยา albendazole หรือ mebendazole; สำหรับการรักษา single abscess คือ ultrasonography-guided drainage with subsequent antihelminthic therapy.

ผู้ป่วยรายนี้ แพทย์ได้เจาะดูด liver abscess under ultrasonographic guidance, และพยาธิได้ออกมาทาง 16F (5.33 mm) pigtail catheter (รูป 2). หลังจากนั้นผู้ป่วยได้รับยา albendazole 200 มก วันละ 2 ครั้งนาน 3 วัน. จากการติดตามหลังจากนั้นผู้ป่วยสบายดี.
