เด็กชายอายุ 3 ปี มีไข้และไอมา 8 วัน ผู้ป่วยมี respiratory distress

เด็กชายอายุ 3 ปี ซึ่งแข็งแรงดีมาก่อน มีไข้และไอมา 8 วัน. ผู้ป่วยไม่มีประวัติ foreign-body aspiration. PE: He had signs of respiratory distress and a low-pitched breath sound on his left lung.
CXR ทำเมื่อ 2 วันก่อน: ปกติ.
CT scan chest วันที่ admit พบ left lower lobe atelectasis (รูป 1).
-Direct immunofluorescence ของ nasopharyngeal swabs ตรวจหา respiratory syncytial virus, influenza A + B viruses, parainfluenza viruses และ adenovirus ได้ผลลบ.
-RT-PCR สำหรับ Mycoplasma pneumoniae ใน nasopharyngeal swabs ได้ผลลบ.

-Cast นี้ประกอบด้วย fibrinous material ซึ่งมี neutrophils และ eosinophils (ย้อมด้วย haematoxylin and eosin รุป 3).

-Bronchoalveolar lavage fluid ได้ถูก analyse เพื่อหาว่ามีไวรัสหรือไม่โดย direct immunofluorescence assay และได้ผลบวกสำหรับ adenovirus. Next-generation sequencing สำหรับ pathogens ใน bronchoalveolar lavage fluid ก็ได้ผลบวกสำหรับ adenovirus B1.

1 วันหลังทำ bronchoscopy, ไข้ลงเป็นปกติ และในวันที่ 3 CXR หายดีเป็นปกติ (รูป 4).

Plastic bronchitis เป็น rare, partial or complete airway obstructive disease อย่างหนึ่งซึ่งมีลักษณะจำเพาะคือ “formation of arborising, thick, tenacious casts of the tracheobronchial tree”. Plastic bronchitis เกิดขึ้นได้ใน clinical settings หลายอย่าง และยังไม่เข้าใจถึงสาเหตุการเกิดอย่างถ่องแท้ ส่วนใหญ่ของเคสที่รายงานเป็นเด็กและสัมพันธ์กับ Fontan operation สำหรับ congenital heart disease. นอกเหนือจากนั้น มีรายงาน plastic bronchitis คนที่เป็นโรค sickle-cell disease, asthma, และ influenza A virus infection.
