หญิงอายุ 21ปี ซึ่งเป็น acute myeloid leukemia ที่กำลังได้รับ induction chemotherapy มีรอยโรคที่เข่าซ้ายซึ่งเกิดขึ้น 5 วันหลังเริ่ม chemotherapy.
PE: afebrile, a 2-cm hemorrhagic patch with an erythematous border (รูป A). Otherwise are within normal limits.
Labs: pancytopenia and absolute neutrophil count of 490 cells/cu mm.

ต่อมารอยโรคลุกลามอย่างรวดเร็วกลายเป็น violaceous pustule (รูป B) และผู้ป่วยมีไข้ 39.5°C. แพทย์ได้ทำ punch biopsy ที่รอยโรคและ histopathology ของ specimen พบ perivascular hemorrhage and infiltration of neutrophilic granulocytes with central necrosis.

การวินิจฉัยคือ ecthyma gangrenosum (bacteremic).

Ecthyma gangrenosum เป็นการติดเชื้อของผิวหนังอย่างหนึ่งที่สัมพันธ์กับ P. aeruginosa bacteremia. การติดเชื้อนี้พบใน immunocompromised หรือ critically ill patients และต้องการการวินิจฉัยและการรักษาอย่างรวดเร็วทันท่วงที.

ผู้ป่วยได้รับการรักษาด้วย ceftazidime และ gentamicin IV. เนื่องจากแพทย์คิดถึง deep infection ผู้ป่วยจึงได้รับการทำ surgical débridement (รูป C).

เมื่อผู้ป่วยผ่านพ้น aplastic phase แล้ว แผลหายได้เอง (รูป D แสดงลักษณะแผลที่ 3 เดือนหลังจาก induction chemotherapy).
ผู้ป่วยไม่ได้มี complete remission ของ acute myeloid leukemia และต่อมา ผู้ป่วยได้รับ allogeneic stem-cell transplantation.
