PE: bronchial breath sounds in the left lower lung; no subcutaneous emphysema was palpated.

CXR showed perihilar opacities in both lungs with free air in the mediastinum (รูป A, arrows). Computed tomography confirmed pneumomediastinum (รูป B, arrows) with enhanced interstitial markings and lower-lobe opacities in both lungs, with greater severity in the left lung.

Throat swab ได้ผลบวกสำหรับ influenza A (H1N1).

ผู้ป่วยได้รับ noninvasive ventilatory support และได้รับ zanamivir สำหรับ influenza และ piperacillin–tazobactam สำหรับ bacterial superinfection.

Pneumomediastinum หายไปโดยไม่ต้องทำ intervention ใดๆ. ผู้ป่วยได้รับอนุญาตให้กลับบ้านในวันที่ 15 ของการอยู่รพ.
