ชาย74 ปี มีเต้านมข้างซ้ายโตมากขึ้นมา 2 เดือน+มีไข้และไอแห้งๆ.




   ชายอายุ 74 ปี มีเต้านมข้างซ้ายโตมากขึ้นมา 2 เดือนร่วมกับมีไข้และไอแห้งๆ.
ตรวจร่างกายพบดังรูป A และฟังได้ crepitation ที่ปอดทั้งสองข้าง.

   Examination showed gynecomastia on the left side (Panel A).
CT of the thorax showed a left-chest-wall collection of fluid with erosion of the ribs and extension into the pleural space, anterior mediastinum, and lung parenchyma (Panel B and Panel C, arrows). There were also bilateral nodular infiltrates, a right pleural effusion, and enlarged mediastinal lymph nodes.

   -ทำ fine-needle aspiration ที่ left chest wall เอา fluid มาย้อม acid fast stain พบ acid-fast bacilli.
-Fluid ที่เจาะได้จาก left chest wall ส่งเพาะเชื้อ ขึ้น Mycobacterium tuberculosis.
Cultures ของ bronchoalveolar-lavage fluid ขึ้น M. tuberculosis, Transbronchial biopsy พบ granulomas.
การวินิจฉัย: Pulmonary tuberculosis with a tuberculous abscess of the left chest wall.

   -ผู้ป่วยได้รับการรักษาด้วย antituberculous drugs ร่วมกับ débridement of the chest-wall abscess.
-หลังการรักษาด้วย antituberculous drugs นาน 9 เดือน CT chest พบ resolution ของ tuberculosis. ผู้ป่วยหายดี.
