ชายอายุ 26 ปีมีไข้ ไอ หอบเหนื่อยเล็กน้อยและมีผื่นมา 5 วัน.




   ชายอายุ 26 ปีมีไข้ ไอและหอบเหนื่อยเล็กน้อยมา 5 วัน. ผู้ป่วยเริ่มมีผื่นขึ้นวันที่ 2 ของไข้. ผู้ป่วยสูบบุหรี่ (30 มวนต่อวันมานาน 14 ปี).
PE: fever, skin examination reveals as in Panel A.

   -ลูกสาวของผู้ป่วยเพิ่งจะเป็นอีสุกอีใส (chickenpox) -ผู้ป่วยไม่เคยได้รับการฉีดวัคซีนป้องกันโรคสุกใส และไม่เคยเป็นโรคนี้.
Skin lesions: a polymorphic rash with vesicles, pustules, and crusty lesions (Panel A).
There was no evidence of central nervous system or liver involvement.

Chest radiography performed at the time of admission showed multiple small nodules in both lungs (Panel B).

   High-resolution computed tomography revealed numerous ill-defined nodules, 1 to 5 mm in diameter. Coalescence and surrounding halos of ground-glass attenuation were also observed in some nodules (Panel C).

Serologic testing for HIV was negative.

   การวินิจฉัย : Varicella pneumonia จากลักษณะผื่น อาการไอ เหนื่อย และ CXR และการ contact กับเด็กที่เป็น chickenpox.

ผู้ป่วยได้รับการรักษาด้วย IV acyclovir. CXR Follow-up วันที่ 7 ของการอยู่โรงพยาบาล พบว่า nodules หายไปหมด. ผู้ป่วยหายเป็นปกติดี.
