เด็กหญิงอายุ 5 ปี มีรอยโรคที่คันที่ขาทั้ง 2 ข้าง

เด็กหญิงอายุ 5 ปี แม่พามาตรวจเพราะมีรอยโรคที่คันที่ขาทั้ง 2 ข้าง. ผู้ป่วยเพิ่งกลับมาจากการเดินทางไป Sierra Leone, Africa. รอยโรคนี้เริ่มเกิดขึ้นเมื่ออยู่ที่นั่นได้ 3 สัปดาห์และมีขนาดใหญ่ขึ้นและเป็นแผล. ผู้ป่วยได้รับวัคซีนครบ. PE : afebrile, an area over the medial aspect of the left lower leg was ulcerated and bleeding (รูป A).
Labs: C-reactive protein level : mildly elevated, WBC: normal.

แพทย์ให้ empirical treatment ด้วย oral dicloxacillin และส่ง skin swab ไปเพาะเชื้อ
-ผล skin swab culture ขึ้นเชื้อ Staphylococcus aureus, group A streptococcus, และ Corynebacterium diphtheriae.
-Gram’s staining : predominantly gram-positive bacilli characteristic of corynebacterium (รูป B).
- Immunodiffusion test (Elek’s test) ได้ผลบวก ซึ่งแปลว่า diphtheria นี้สร้าง toxin.

อาการของ cutaneous diphtheria ระยะแรกปรากฎเป็น nonhealing, well-demarcated ulcers.

หลังพบเชื้อ diphtheria ในผู้ป่วยรายนี้ แพทย์เปลี่ยนยาปฏิชีวนะเป็น clarithromycin. ได้ทำ contact tracing และให้ chemoprophylaxis แก่ household contacts.

การติดตามที่ 1 สัปดาห์หลังเริ่มให้ clarithromycin รอยโรคที่ผิวหนังหายดี.
