   ชายอายุ 45 ปี มาพบแพทย์เนื่องจากมีไข้ ไอ และมีแผลที่คางมานาน 3 เดือน. ผู้ป่วยเบื่ออาหาร กินได้น้อยลง น้ำหนักตัวลดลง 8 กก.ใน 3 เดือน
PE: thin, moderately cachectic, alert, T 38.5oC
There was a 5 × 5-cm superficial ulcer with a thick adherent crust over the chin (รูป 1) and three papules with central umbilication over the neck and face.
Liver+spleen were not palpable.

   histopathology ของ umbilicated papule และแผล รูปไม่ช้ดค่ะ พบ numerous yeast forms and a few sausage-shaped structures with central septation within the histiocytes

   Fungal tissue cultures จาก skin lesions และ bronchoalveolar lavage aspirates ขึ้นเป็น greenish yellow, flat, velvety wrinkled colony with surrounding diffusible red pigment (รูป 3).

การวินิจฉัยคือ การติดเชื้อ Penicillium marneffei.

Sputum smears และ culture ได้ผลลบสำหรับเชื้อวัณโรค.
Direct immunofluorescence detection สำหรับ Pneumocystis jirovecii จากเสมหะได้ผลลบ.

   คนไข้รายนี้ HIV antibody positive ค่ะ

Mucocutaneous lesions พบใน 2/3 ของผู้ป่วยที่เป็นโรคติดเชื้อ Penicillium marneffei. ลักษณะรอยโรคที่พบเป้น generalized papular eruptions, central umbilicated papules คล้าย molluscum contagiosum, acne-like lesions, folliculitis, subcutaneous nodules, necrotic nodules, papules and nodules with ulceration, oropharyngeal papules, palatal perforation, และ genital papules.
