ชายอายุ 32 ปี เพิ่งกลับมาจากไนจีเรีย ไอแห้งๆมา 3 สัปดาห์ร่วมกับมีไข้และอ่อนเพลีย.

ชายอายุ 32 ปี ซึ่งเพิ่งกลับมาจากการไปทำงานที่ไนจีเรีย มาพบแพทย์เนื่องจากไอแห้งๆมา 3 สัปดาห์ร่วมกับมีไข้และอ่อนเพลีย. เขาไปอยู่ที่ไนจีเรียนาน 6 เดือนซึ่งที่นั่นเขาว่ายน้ำในแหล่งน้ำจืดบ่อยๆ. CBC: WBC 8,399/mm3, eosinophils 18.8%, absolute eosinophil count 1,579/mm3) และ broncho-alveolar lavage fluid พบ eosinophil 15%. Chest computed tomography (CT) showed bilateral micro- and macronodules with ground-glass halos (Figure 1).

Pantip Chayakul: Schistosomiasis เป็นโรคติดเชื้อ helminth ชนิดหนึ่งซึ่งมีถิ่นของโรคใน tropical และ subtropical regions. คนได้รับพยาธินี้จากการสัมผัสโดยตรงกับ contaminated fresh water. Early schistosomiasis สามารถเกิดขึ้นที่ 14 วัน หลัง primary infection หรือ heavy re-infection และแสดงถึง immunogenic response ต่อ migrating schistosomae cercariae ก่อนที่พยาธิจะวางไข่. Pulmonary phase นี้พบได้บ่อยโดยที่ผู้ป่วยอาจไม่มีอาการใดๆก็ได้ และเกิดได้จาก schistosomasis species (ที่พบบ่อยที่สุดคือ S. mansoni and S. japonicum). ผู้ที่มีภาวะนี้เกือบทั้งหมดเป็น non-immune travelers ใน schistosomiasis hematobium และ mansoni.
Computed tomography เป็นการตรวจที่ไวที่สุดและแสดงลักษณะ typical คือ pulmonary micro- or macronodules surround by ground glass halo. Ground-glass halo นี้อาจสัมพันธ์กับ eosinophilic lung parenchyma infiltration หรือ immune complex deposition.
