ชายอายุ 33 ปีเปวดท้องส่วนบนรุนแรงมา 2 วันและมี eosinophilia 29%


   ชายอายุ 33 ปีเข้ารับการรักษาในโรงพยาบาลเนื่องจากปวดท้องส่วนบนแบบปวดบีบๆเป็นพักๆ มา 2 วัน ปวดรุนแรงและกินอาหารได้น้อย. Physical examination was positive for tenderness to palpation in the right upper quadrant with involuntary guarding and rebound tenderness. Laboratory tests ปกติ ยกเว้น CBC: WBC 16,800/cu mm with 54% neutrophils and 29% eosinophils (absolute 4,650/cu mm).
Abdominal films were concerning for an ileus.
CT showed a normal appendix, wall thickening of the terminal ileum, and a possible closed loop obstruction.

    ศัลยแพทย์ได้รับปรึกษาและได้ทำ emergent exploratory laparotomy ซึ่งพบ inflammation of the terminal ileum และ ไม่พบว่ามี closed loop obstruction.
Colonoscopy revealed erythema in the terminal ileum and a worm measuring 7 mm attached to the mucosa (รูป). Tissue biopsies of the terminal ileum were negative for inflammatory bowel disease.
น่าจะเป็นพยาธิอะไรคะ

   พยาธิถูกดึงออกมา และได้ถูกส่งไป identified โดย clinical laboratory scientist พบว่าเป็น adult female ของ species Ancylostoma duodenale.
หลังจากเอาพยาธิออกไป ผู้ป่วยได้รับการรักษาด้วย mebendazole 100 mg วันละ2 ครั้งนาน 3 วัน.
แพทย์ได้อนุญาตให้ผู้ป่วยกลับบ้านวันที่ 5 ของการอยู่โรงพยาบาลโดยผู้ป่วยหายปวดท้องและ eosinophil count ลดลงเป็น 15% ((absolute 1,530/cu mm).

   มี 2 main species ของ hookworms ที่ infect คนและมีคนเป็นผู้แพร่พยาธินี้ คือ Necator americanus และ Ancylostoma duodenale. ยังมี species อื่นๆ ที่พบในสุนัข แมว หรือหมูสามารถ infect คนได้ ซึ่งเรียกว่า zoonosis.

อาการของ hookworm infection ขึ้นอยู่กับ stage ของ infection ใน human host. Hookworm larva สามารถทำให้เกิด pruritic rash เมื่อไชผ่านผิวหนังเข้าไป (เรียกว่า ground itch). พยาธิตัวอ่อนเดินทางทาง blood circulation ไปสู่ปอด แทรกเข้าไปในเนื้อปอด (Loeffler syndrome) แล้วเดินทางไปสู่ bronchial tree ซึ่งทำให้ผู้ป่วยไอและเจ็บคอ. อาการที่พบใน acute hookworm infection มากที่สุด เกิดขึ้นเมื่อ larva เดินทางไปถึง GI tract, ลอกคราบไปเป็น adult worm และเกาะติดกับ mucosa. อาการเหล่านี้คือ cramping, abdominal pain, nausea, diarrhea และ flatulence. การเป็นโรคนี้อยู่นานมักทำให้เกิดซีดจาก chronic blood loss. ยาที่นิยมใช้รักษา intestinal hookworm infection คือ mebendazole, albendazole หรือ pyrantel pamoate.

   จากอาการ และ endoscopic findings, helminthic infection สาเหตุที่น่าจะเป็นมากที่สุดในผู้ป่วยรายนี้ซึ่งมาด้วย acute abdominal presentation. ในผู้ป่วยที่สัมผัสกับสุนัข ต้องนึกถึง zoonotic infection จาก dog hookworm A. caninum ไว้ด้วย. A. caninum เป็นสาเหตุของ eosinophilic enteritis ในผู้ป่วยหลายรายใน Australia.
