หนึ่งสัปดาห์ก่อนหน้านั้น ผู้ป่วยได้รับการถอนฟันผุที่กรามล่างด้านขวา 2 ซึ่.

Physical examination showed he had a fever (39•5°C), painful swelling of the right cheek, and proptosis and chemosis of both eyes (figure).

diagnosis: septic cavernous sinus thrombophlebitis ที่เกิดตาม dental infection เชื้อก่อโรคน่าจะเป็นเชื้อ flora ในช่องปาก คือ viridans streptocooci และ anaerobe
management
1. Investigation
- CBC, hemoculture
- CT หรือ MRI brain + sinuses
- aspirate cavernous sinus เอาหนองมาย้อมสี gram และส่งเพาะเชื้อ
2. Antibiotic และ drainage
antibiotic ขอเลือก ceftriaxone+ metronidazole ค่ะ

คุณหมอ Daisy 1 ตอบถูกต้องแล้วค่ะ เห็นด้วยกับคุณหมอเรื่อง antibiotic ที่เลือกใช้กับผุ้ป่วยรายนี้
ค่ะ

Laboratory tests:
WBC 12,200/cu mm และ C-reactive protein concentration = 2629 nmol/L.
MRI : several facial, intraorbital, and intracranial abscesses with bilateral cavernous sinus thrombosis.

ผู้ป่วยได้รับการรักษาด้วย IV antibiotics และ surgical drainage abscesses.
Bacterial cultures ขึ้น Streptococcus anginosus.

ผู้ป่วยมีอาการดีขึ้นหลัง 3 สัปดาห์ และหายเป็นปกติ.

Septic cavernous sinus thrombosis ที่เกิดจาก dental infection พบได้น้อยแต่รุนแรงถึงชีวิตได้. ผู้ป่วยส่วนใหญ่ที่มี cavernous sinus thrombosis มี proptosis และ chemosis. การแพร่กระจายไปสู่ตาอีกข้างหนึ่งเป็นลักษณะทางคลินิกที่พบบ่อยของ cavernous sinus thrombosis และมักเกิดขึ้นภายใน 24–48 ชั่วโมง. การรักษามุ่งไปที่ primary infection และควรพิจารณาการใช้ anticoagulants ร่วมด้วย.
