หญิงอายุ 70 ปี มี black hyperpigmentation of the shins and sclera




   หญิงอายุ 70 ปี ได้เข้ารับการรักษาในโรงพยาบาลเนื่องจากมี symptomatic atrial fibrillation. ผู้ป่วยมีประวัติการเป็น oxacillin-resistant coagulase-negative staphylococcal endocarditis และมีภาวะแทรกซ้อนเป็น aortic root abscess จึงได้รับการผ่าตัดและ aortic valve re-replacement ด้วย bioprosthesis เมื่อ 5 ปีก่อน. หลังจากนั้น ผู้ป่วยได้รับ long-term suppressive antibiotic therapy ด้วย ยาปฏิชีวนะชนิดหนึ่ง มาตลอด.
On examination, she had black hyperpigmentation of the shins and blue–black scleral hyperpigmentation (figure).

   ยากินสำหรับรักษา MRSA infection ซึ่งจะครอบคลุม MRSE ด้วยดังในตาราง

ยาที่มี adverse effect ทำให้เกิด brownish or bluish-black pigmentation of the skin คือ minocycline ค่ะ

diagnosis: Hyperpigmentation secondary to minocycline

รูป (A) Black hyperpigmentation of the legs.
(B) Blue–black hyperpigmentation affecting both sclerae, more prominent on the right

ทั้งที่ผู้ป่วยมี dyschromia, ผู้ป่วยยังคงได้รับ minocycline ไปตลอดชีวิตเนื่องจากมีความเสี่ยงเรื่อง previous endocarditis. ผู้ป่วยได้รับการทำ electrical cardiovertion สำหรับ symptomatic atrial fibrillation.

แม้ว่ายังไม่ทราบ mechanism ของการเกิด hyperpigmentation โดย minocycline, minocycline เมื่อถูก oxidize จะเปลี่ยนเป็นสีดำ. ยานี้สามารถทำให้สีของผิวหนัง, bulbar conjunctiva, เล็บและฟัน เปลี่ยนได้. แสงแดด และการใช้ยา estrogen, amitriptyline และ phenothiazines ร่วมกับ minocycline สามารถทำให้เกิด hyperpigmentation มากขึ้น.
Resolution ของ hyperpigmentation ใช้เวลานานหลายเดือนถึงหลายปีหลังหยุดยา แต่ก็อาจคงอยู่ไปตลอดได้.
