หญิงอายุ 33 ปี มีไข้ ไอ และหอบเหนื่อยมา 2 วัน และมีผื่นมา 1 สัปดาห์.

หญิงอายุ 33 ปีมาที่ห้องฉุกเฉินเนื่องจากมีไข้ ไอ และหอบเหนื่อยมา 2 วัน และมีผื่นคันมา 1 สัปดาห์.
On examination she had a temperature of 41.0°C, blood pressure of 98/65 mm Hg, heart rate of 85 bpm, respiratory rate of 16 bpm, and O2 saturation of 47% on room air. She had a diffuse rash affecting the trunk, extremities, face, and scalp, but sparing the palms and soles (ดังในรูป). She had normal mental status, cranial nerves, motor, and sensory examination.
Chest radiograph showed diffuse bilateral airspace opacities.

หลังจากนั้น 3 ชั่วโมง ผู้ป่วยสับสน มี flaccid paralysis และต้องใส่ ET และได้รับผู้ป่วยเข้า medical ICU.

Electroencephalogram: non-convulsive status epilepticus.

MRI brain: multiple foci of punctate microhaemorrhages throughout the corpus callosum, both temporal lobes, and left posterior cingulate gyrus.

Pertinent data ของผู้ป่วย
1. ไข้และ diffused skin lesion มี multiple stages lesions เป็น macules, papules และ lesion ที่มี scab ตรงกลางเหมือนเคยเป็น vesicle แล้วแตก นึกถึง chickenpox มากที่สุดค่ะ
2. ไอ และหอบเหนื่อย, O2 saturation of 47% on room air. Chest radiograph showed diffuse bilateral airspace opacities. นึกถึง severe varicellar pneumonia มากที่สุดค่ะ ผู้ป่วยมี severe hypoxemia.
3. ผู้ป่วยสับสน มี flaccid paralysis, EEG: non-convulsive status epilepticus และ MRI brain: multiple foci of punctate microhaemorrhages throughout the corpus callosum, both temporal lobes, and left posterior cingulate gyrus เข้าได้กับ encephalitis ซึ่งรายนี้ก็น่าจะเกิดจาก Varicella-zoster virus

สรุปว่าผู้ป่วยน่าจะมี varicella pneumonia and encephalitis ซึ่งก็ต้อง investigate หาว่าเป็น immunocompromised host หรือไม่

Further investigation
1. ส่งเลือดไปตรวจหา HIV antibody
2. ส่ง skin biopsy ทำ PCR for VZV
Rx ให้ IV acyclovir เลยค่ะ
Respiratory care ให้ดีค่ะ

ผู้ป่วยมี diffuse papular rash with vesicles of different ages with haemorrhagic crust affecting the trunk, extremities, face, and scalp, but sparing the palms and soles (figure).
-MRI consistent with infectious haemorrhagic encephalitis.
-HIV testing positive, with CD4 count 4 cells/μL (normal 348–1456 cells/μL), และ viral load was 66 000 copies/mL (normal < 50 copies/mL).
-Skin biopsy sample stained positive for varicella zoster virus (VZV), but negative for herpes simplex virus (HSV).

Zoster-associated encephalitis จาก disseminated VZV สามารถเกิดขึ้นได้ภายในไม่กี่วันหลังเริ่มมีผื่น และต้องนึกถึงอย่างมาก ว่าผู้ป่วยมีภูมิต้านทานต่ำ. อาการแสดงที่เป็นลักษณะจำเพาะของ zoster-associated encephalitis ประกอบด้วย acute delirium, typical signs ของ stroke เช่น headache with acute hemiplegia, aphasia, ataxia, hemisensory loss, หรือ visual changes โดยขึ้นอยู่กับ predominant location ของ VZV vasculitis. การรักษาต้องเริ่มให้อย่างรวดเร็วเมื่อผู้ป่วยมีลักษณะทางคลินิกที่ทำให้แพทย์นึกถึง encephalitis ด้วย intravenous acyclovir 10 mg/kg ทุก 8 ชั่วโมงนาน 7–10 วัน.

ผู้ป่วยรายนี้ได้รับ IV aciclovir และยากันชักหลายตัวเพื่อควบคุมอาการชัก ผู้ป่วยได้รับการรักษา VZV pneumonia, aspergillus pneumonia, propofol-induced pancreatitis, Escherichia coli urinary tract infection และ critical illness myopathy และได้รับ pneumocystis pneumonia prophylaxis. เมื่อผู้ป่วยมีอาการดีขึ้น แพทย์จึงเริ่มให้ antiretrovirals.
หลังทำกายภาพบำบัดนาน 2 เดือน ผู้ป่วยสามารถเดินได้เองและสื่สารกับผู้อื่นได้
การติดตามผู้ป่วยที่ 6 เดือนต่อมา cognitive และ motor function ของผู้ป่วยดีจนเกือบเป็นปกติ.
