A 46 YOF with HIV infection had painful rash at leg




   หญิงอายุ 46 ปี ติดเชื้อ HIV มาที่ ER เนื่องจากมีผื่นที่ขาขวาดังในรูป มีอาการปวดที่ผื่นนี้ ไม่คัน

เมื่อ 3 สัปดาห์ก่อน ถูกแมวข่วนที่ตำแหน่งนี้ แล้วได้รับการรักษาด้วยยา amoxicillin–clavulanate อยู่นาน 14 วันโดยไม่ดีขึ้น

เรื่องติดเชื้อ HIV ผู้ป่วยยังไม่ได้รับยาต้านไวรัส
CD4 cell count ล่าสุด 365/cumm.

PE: ผู้ป่วยดูดี ไม่มีไข้
A violaceous plaque that measured 9 cm by 9 cm and consisted of four concentric rings with intervening areas of normal skin and numerous yellow-white pustules was found on the pretibial surface of the right leg. At the periphery of the rings, there was fine scaling of the skin.

2. จะ manage อย่างไร

   - คิดว่าเป็น Tinea Circinata ค่ะ
- management; scrap lesion แล้วหยด KOH ดูด้วยกล้องจุลทรรศ เพื่อหา hyphae ค่ะ

   - Tinea gladiatorum
สาเหตุจากเชื้อ Trichophyton tonsurans
การรักษา fluconazole 200 mg weekly for 4 weeks หรือเปล่าครับ

   การรักษา Tinea circinata
1. Topical antifungal
- Clotrimazole 1% cream
2. Systemic Antifungal
Indication; Immunocompromised patient
- Fluconazole 150 mg PO once per week x 4 weeks or
- Itraconazole
