ชายอายุ 43 ปี มีไข้และเจ็บที่คอด้านหน้าข้างซ้ายมา 2 วัน.

ชายอายุ 43 ปี มีไข้และเจ็บที่คอด้านหน้าข้างซ้ายมา 2 วัน. เมื่อ 2 ปีก่อน ผู้ป่วยได้รับการปลูกถ่ายไต ( deceased-donor kidney transplant). ผู้ป่วยมี chronic renal allograft rejection และ chronic hepatitis C infection. ยาที่ผู้ป่วยได้รับอยู่คือ mycophenolate mofetil, prednisolone, และ tacrolimus.

PE his left anterolateral neck was swollen, warm, and tender.

CBC: white blood cell count 21,800/mm 3 with a neutrophil count of 19,400 /mm 3.

Computed tomography scan of the neck showed a 3-cm × 3-cm × 9-cm complex fluid collection in the left thyroid lobe that extended superiorly to the submandibular region, inferiorly to the clavicular head, and medially to the isthmus ( รูป A และ B, arrows). The left carotid sheath was displaced laterally and a mild mass effect was evident on the trachea.

Problem list
1 Fever with neck pain
2 immunocompromised patients
2.1 chronic renal allograft rejection และ chronic hepatitis C infection
2.2 : on immunosuppressive drugs ( mycophenolate mofetil, prednisolone, tacrolimus. )
3 CBC : Leukocytosis with PMN predominate
4 CT neck showed a 3-cm × 3-cm × 9-cm complex fluid collection in the left thyroid lobe
Diagnosis : thyroid abscess (suspected S.aureus)
Management :
- U/S guide drainage+ sent specimen for culture
- Start ATB cefazolin for cover gram positive gram negative
- Beware airway compromise

Blood cultures และ specimens ของ thyroid fluid collection ขึ้น methicillin-susceptible Staphylococcus aureus.

Thyroid abscess พบได้น้อย คือน้อยกว่า 1% ของโรคของ thyroid ทั้งหมด. การที่พบน้อยเนื่องจาก thyroid gland มี ปัจจัยปกป้องตัวเอง คือ high iodine content, capsular encasement และ good lymphatic drainage. Thyroid abscess พบได้บ่อยที่สุดในผู้ป่วยที่มีความผิดปกติของไทรอยด์อยู่แล้ว เช่น nodules หรือ cancer; anatomic defects, เช่น pyriformis sinus fistulae หรือ compromised immune systems ดังเช่นผู้ป่วยรายนี้. ฝีอาจเกิดขึ้นจาก hematogenous หรือ lymphatic spread หรือ direct spread จากการติดเชื้อที่คอหรือ oropharynx. ที่พบน้อย คืออาจเป็นผลจาก trauma หรือ direct inoculation ระหว่างการทำ invasive procedures เช่น fine-needle aspiration ของ thyroid gland หรือ central venous line placement. ภาวะแทรกซ้อนของ thyroid abscess ได้แก่ tracheal หรือ esophageal perforation, descending necrotizing mediastinitis, extension เข้าสู่ deep spaces ของคอและทำให้ผู้ป่วยเสียชีวิตได้.
เชื้อก่อโรคที่ทำให้เกิด thyroid abscess ที่พบบ่อยที่สุด คือ S. aureus. เชื้ออื่นๆ ได้แก่ anaerobes ของ oropharyngeal area หรือ Gram-negative aerobes.
การรักษา thyroid abscesses คือ drainage หรือทำ partial หรือ total thyroidectomy ร่วมกับการให้ยาปฏิชีวนะที่เหมาะสม. ถ้าผู้ป่วยมี pyriformis sinus tract ก็แนะนำให้ทำ resection หรือ obliteration ของ tract. ในผู้ป่วยรายนี้ thyroid abscess หายจากการทำ percutaneous drainage และ cefazolin therapy.
