ชาย 76 ปี อาเจียน ปวดแน่นท้อง และไม่ถ่ายอุจจาระมา 3 วัน.




   ชายอายุ 76 ปีซึ่งมีโรค diffuse B-cell non-Hodgkins lymphoma และ chronic neutropenia and thrombocytopenia มาโรงพยาบาลเนื่องจากมีอาเจียน ปวดแน่นท้อง และเพลียมา 3 วัน. ผู้ป่วยไม่ถ่ายอุจจาระมา 3 วัน.
On examination, the patient was pyrexial and showed signs of septic shock. The abdomen was distended and a paraumbilical hernia present.
-Blood tests showed lactic acidosis, low white blood cell count, and elevated urea, thus confirming a diagnosis of septic shock.

   Problem list
1 Aging man u/d diffuse B-cell non-Hodgkins lymphoma . chronic neutropenia and thrombocytopenia
2 abdominal pain , vomiting , ไม่ถ่าย 3 วัน
PE :abdomen was distended and a paraumbilical hernia present.
>>suspected sign gut obstruction ??
3 septic shock (lactic acidosis ,low WBC ,elevated urea)
4 CT abdomen (อ่านเอง)
panel A air at gastric mucosa and portal vein
panel B strangulated umbilical hernia

Imp : Gastric necrosis and mesenteric ischemia

Mx :
- for septic shock treat as early directed goal therapy empirical ATB with fortum and amikin
- for gastric necrosis and mesenteric ischemia
emergency Sx for gastractomy + jejunostomy

CT scan of the abdomen showed peeling of the gastric mucosa and air in the portal vein (รูป A).
การวินิจฉัย: acute gastric necrosis secondary to acute gastric dilatation and obstruction caused by a strangulated para umbilical hernia (รูป B, white arrow)

ผู้ป่วยได้รับการผ่าตัด emergency total gastrectomy และ ใส่ feeding jejunostomy.
หนึ่งเดือนต่อมา ผู้ป่วยได้รับการผ่าตัด Roux-en-Y reconstruction.

จากการติดตามหลายเดือนต่อมา ผู้ป่วยกินอาหารวันละ 3 มื้อ และน้ำหนักคงที่.
