1. จงอ่าน blood smear นี้
2. บอก differential diagnosis

   ความผิดปกติที่พบใน peripheral blood smear นี้ คือ

-Normochromic normocytic anemia with poikilocytosis
-Teardrop-shaped red cells
-Nucleated red cells (normoblast)
-Low-grade reticulocytosis
-Metamyelocytes, Myelocyte
-Occasionally promyelocyte, myeloblast
-Thrombocytopenia, Giant platelets

ลักษณะทั้งหมดนี้ เรียกว่า Leukoerythroblastic (myelophthisic) blood picture

ซึ่งเกิดจาก
1. Increase demand
2. Bone marrow disease

-Hemorrhage
-Hemolysis
-Severe infection
-Recovery from bone marrow failure or suppression
-Sickle cell crisis
-Thalassemia major

   Bone marrow disease พบใน

-Primary myelofibrosis
-Hematologic malignancy : CML, AML, ALL, HL, NHL, MM
-Metastatic cancer : lung, breast, prostate cancer
-Granuloma : miliary tuberculosis, fungal infection
-Bone marrow infarction
-Macrophage proliferation in storage disease : Gaucher disease -Severe megaloblastic anemia
-Severe rickets

   แนวทางการ aaproach ผู้ป่วยที่มี Leukoerythroblastic (myelophthisic) blood picture

1. History and physical examination base on underlying disease
2. Skeletal radiography : lytic and blastic lesion  prostate, lung, breast cancer metastasis
3. Bone scan : high sensitivity
4. Bone marrow aspiration : dry tap in marrow infiltrative disease
5. Bone marrow biopsy
