หญิงอายุ 54 ปีมาโรงพยาบาลด้วยอาการปวดท้องข้างซ้ายส่วนล่างมา 5 วัน ร่วมกับมีไข้.
white blood cell count เมื่อแรกรับเข้ารพ. 19.1 x 103 cells/mm3.
ผู้ป่วยมีประวัติเป็น diverticulosis อยู่เดิม.
Computed tomography (CT) scan of the abdomen with intravenous (IV) and oral contrast revealed an area of fat stranding adjacent to diverticulosis of the proximal sigmoid colon. This finding was consistent with acute diverticulitis.

คำถาม ยาปฏิชีวนะที่เหมาะสมสำหรับผู้ป่วยรายนี้คืออะไร

antibiotic สำหรับ diverticulitis
การติดเชื้อใน diverticulitis เกิดจากเชื้อ aerobic Gram-negative bacilli ในกลุ่ม enterobacteriaceae คือ E. coli เป็นตัวสำคัญ และ anaerobes ตัวสำคัญคือ Bacteroides fragilis ยาปฏิชีวนะที่เลือกใช้ต้องครอบครุมเชื้อสองตัวนี้ คือ
1. Ceftriaxone+Metronidazole หรือ
2. Amoxicillin+clavulanic acid (Augmentin)

คุณหมอ Daisy1 ตอบถูกแล้วค่ะ

ผู้ป่วยเริ่มได้รับการรักษาด้วย IV ciprofloxacin และ IV metronidazole แต่อาการของผู้ป่วยไม่ดีขึ้น 4 วันต่อมา ผู้ป่วยปวดท้องมากขึ้น ต้องใช้ยาแก้ปวดขนาดสูงขึ้น
(สำหรับเชื้อ E. coli ปัจจุบันดื้อต่อ fluoroquinolone มากขึ้น เชื้อจะไวต่อ ceftriaxone มากกว่า ciprofloxacin ค่ะ)

แพทย์ได้ส่ง CT scan abdomens ซ้ำ คราวนี้ไม่มี contrast
1. พบความผิดปกติอะไรในรูปนี้
2. จะ manage อย่างไร

จาก CT abdomen-non contrast:
พบมี structure โค้งๆ ที่มี bone density ซึ่งน่าจะเป็น foreign body อยู่ในลำไส้ ค่ะ
Management: colonoscopy เพื่อ remove foreign body

CT without contrast : disclosed an area of fat stranding adjacent to the diverticulosis of the proximal sigmoid colon. In addition, curvilinear dense calcifications within the sigmoid colon lumen were evident, raising concern for a foreign body (รูป 1). Tiny flecks of extraluminal air were noted within the region. No abscess or fluid collection was identified.

หลังจากนั้น ผู้ป่วยได้รับการตรวจด้วย flexible sigmoidoscopy พบว่ามี multiple large-mouthed diverticula อยู่ภายใน sigmoid colon. ใน diverticula อันหนึ่งมีกระดูกไก่ความยาว 1.5 นิ้วอยู่และปลายหนึ่งโผล่เข้ามาใน sigmoid colon.
แพทย์ได้คีบกระดูกไก่ออกมา พบว่าปลายด้านหนึ่งของกระดูกทำให้เกิดรูทะลุที่ผนังของ diverticulum จึงใช้ clip ปิดรูทะลุนั้น

อาการปวดท้องของผู้ป่วยดีขึ้นอย่างมากภายใน 24 ชั่วโมงหลังทำ flexible sigmoidoscopy และผู้ป่วยไม่ต้องการยาแก้ปวดเลย. วันรุ่งขึ้น ผู้ป่วยกินอาหารเหลวได้และแพทย์อนุญาตให้กลับบ้านโดยกินยาปฏิชีวนะต่อให้ครบ 10 วัน.

เมื่อเอา CT scan อันแรกมาดูซ้ำ พบว่าเห็น linear high-density area อยู่ที่ตำแหน่งเดียวกันภายใน sigmoid colon แต่มองเห็นไม่ชัดเนื่องจากมี oral contrast อยู่ภายใน colon.
