ชาย 25 ปี มีตุ่มนูนที่แก้ม จมูกและติ่งหู และขนคิ้วร่วงทั้งสองข้าง




   ชายอายุ 25 ปีมาหาแพทย์ด้วยเรื่องมีตุ่มนูนสีซีดๆที่แก้ม จมูกและติ่งหูทั้งสองข้างซึ่งค่อยๆโตขึ้นโดยเป็นมานานหลายปีแล้ว ระยะหลังขนคิ้วทั้งสองช้างร่วงไป (รูป 1) ผู้ป่วยยังสบายดีและทำงานได้เป็นปกติ.

1. การวินิจฉัยคือโรคอะไร
2. จะ manage อย่างไร

   1. การวินิจฉัยคือ lepromatous leprosy ค่ะ
2. ทำ skin biopsy และย้อม acid fast ซึ่งจะพบ acid fast bacilli
ถ้าเป็น leprosy ก็ให้การรักษาด้วย rifampicin, dapsone และ clofazimine

   Skin biopsies confirmed the clinical suspicion of granulomatous infiltrates.
Ziehl-Neelsen stains of the tissue showed multiple acid-stable rods. These rods formed cigar-like clusters, which are typical for an
infection with Mycobacterium leprae (รูป B).
The bacterial index was high with about 50 mycobacteria per
high power field.
PCR targeting the internal transcribed spacer (ITS) region of Mycobacterium spp and subsequent DNA sequencing confirmed the diagnosis of leprosy.

ผู้ป่วยได้รับการรักษาด้วยยาสูตร 12 เดือนสำหรับ multibacillary leprosy ตาม WHO guidelines ด้วยยา rifampicin 600 mg เดือนละครั้ง, dapsone 100 mg/วัน และ clofazimine 50 mg/วัน และ 300 mg เดือนละครั้ง.

   หลังเริ่มได้รับการรักษานาน 2 เดือน ขนคิ้วของผู้ป่วยเริ่มงอกและ nodules ขนาดเล็กลง (รูป C).

   ลักษณะทางคลินิกของผู้ป่วยรายนี้มีชื่อเรียกว่า leonine facies. ในประเทศที่พัฒนาแล้ว เมื่อพบผู้ป่วยที่มี leonine facies ก็จะนึกถึง lymphoma หรือ sarcoidosis (ซึ่งมีชื่อเรียกว่า lupus pernio). ส่วนในประเทศกำลังพัฒนา ลักษณะนี้จะเป็น leprosy มากกว่า.

WHO classifi cation แยก leprosy เป็น paucibacillary leprosy ซึ่งมี low bacterial index และ hypopigmented anaesthetic macules จาก multibacillary leprosy ซึ่งมี medium-to high bacterial index และรอยโรคเป็น nodules และ infiltrated plaques.
