ผู้ป่วยหญิงไทยคู่ อายุ 65 ปี อาการสำคัญ เหนื่อยหอบมา 4 วัน


   ผู้ป่วยหญิงไทยคู่ อายุ 65 ปี อาการสำคัญ เหนื่อยหอบมา 4 วัน ประมาณปีเศษๆ ก่อนมาโรงพยาบาล ผู้ป่วยสังเกตว่าแขนขาทั้ง 2 ข้างมีแรง น้อยลงเวลาจะใช้มือจับอะไรจะลำบาก แขนสั่นๆ กล้ามเนื้อแขนและขาลีบลง บางครั้ง กล้ามเนื้อ ที่โคนขากระตุก ผู้ป่วยได้ไปรักษาที่โรงพยาบาลจังหวัดใกล้บ้านแพทย์บอกว่ารักษาไม่ได้ ผู้ป่วยจึงไปซื้อยาลูกกลอนรับประทานเองอาการไม่ดีขึ้น แขนขากลับอ่อนแรงมากขึ้น มือทั้ง 2 ข้างดูเหี่ยวแฟบและสั่น เวลาเดินกระดกเท้าไม่ได้ หลังเริ่มไม่สบายเกือบปีก็เดินไม่ได้เลย 4 วันก่อนเริ่มเหนื่อยมากขึ้น หายใจหอบๆ ญาติจึงพามารักษาที่กรุงเทพฯ
ผู้ป่วยมีประวัติเป็นแรงดันเลือดสูงมา 1 ปี ได้รับประทานยารักษามาตลอด.
คำถาม
1.Problem list ในผู้ป่วยรายนี้คืออะไร
2.จากประวัติท่านสามารถให้การวินิจฉัยเบื่องต้นได้อย่างไร
3.ท่านหวังว่าจะพบอะไรในการตรวจร่างกายที่ผิดปรกติ

Problem list หญิงไทยคู่ อายุ 65 ปี
1. Dyspnea 4 days
2. Progressive muscle weakness of all extremities 1 yr. with muscular atrophy and fasciculation + spasticity, foot drop
3. Steroid usage
4. Hypertension 1 yr.

Problem ของผู้ป่วยรายนี้ทั้งหมดคิดว่าอธิบายได้จากโรค amyotrophic lateral sclerosis (ALS) ซึ่งมี clinical hallmark คือ combination ของ upper motor neuron และ lower motor neuron signs + symptoms.
-Upper motor neuron findings ของ weakness, hyperreflexia และ spasticity เป็นผลจาก degeneration ของ frontal motor neurons.
-Lower motor neuron findings ของ weakness, atrophy หรือ amyotrophy และ fasciculations เป็น direct consequence ของ degeneration ของ lower motor neurons ใน brainstem และ spinal cord.
-ผู้ป่วยมีความดันโลหิตสูงหรือ fluctuation ได้จาก sympathetic hyperactivity
-ส่วนอาการเหนื่อยหอบ 4 วันนี้ เกิดขึ้นได้จาก progressive neuromuscular respiratory failure ซึ่งเป็นสาเหตุสำคัญที่ทำให้ผู้ป่วยโรคนี้เสียชีวิต

   T 36.8ºC PR 108 regular RR 28 BP 140/70
alert, rapid laboured breathing with accessory muscles
Heart sounds : normal
Lungs : clear
Speech : soft, slurred
Tongue : could not be protruded, wasting and fasciculation
were present
Upper Limbs : moderate wasting and weakness of deltoids,
biceps, triceps and fore arm muscles of both sides
marked wasting and weakness of small muscles of
both hands.
Fasciculation ++ in deltoids
Lower Limbs : moderate wasting and weakness of quadriceps,
gastrocnemius, anterior tibial and peroneal muscles
of both sides
slight increased in tone of both legs
Reflexes : hyperreflexia
Sensation in all modalities : normal
