A 35 YOM had fever+sore throat, for 3 days with painful swollen neck and dyspnea




   
A 35-year-old woman presented with a three-day history of sore throat, painful swelling of the neck, fever, rigor, hemoptysis, and dyspnea.

Examination revealed pustular exudates on the right tonsil.

Questions
1. Panel A is the picture of right side of his neck. What is the abnormality?
2. Computed tomographic imaging of the chest was performed (Panel B). What are the abnormalities?
3. What is the most likely diagnosis?
4. Which investigations you will do?

   คุณหมอ Nongnham ได้e-mail มาหาดิฉันเพราะพยายามตอบเข้า e-consult อยู่หลายครั้งตั้งแต่เมื่อวานบ่าย และเช้าวันนี้ ก็ post เข้าไม่ได้ ขอขอบคุณในความพยายามและสำหรับคำตอบค่ะ

1. ภาพแรกที่ลูกศรชี้คือ Right Internal jugular vein โป่งมาก
2. CT มี nodule ในปอดทั้งสองข้าง (Rt 2, Lt 1) และข้างๆ nodule ที่ปอดด้านขวา และเหนือ pleural effusion ที่ปอดข้างซ้าย มีลักษณะเหมือน consolidation ร่วมด้วยค่ะ และมี pleural effusion ทั้งสองข้าง
3. เริ่มแรกเป็น peritonsillar abscess แล้วเชื้อ spread เข้า internal jugular vein เกิด thrombophlebitis การติดเชื้อแพร่ไปสู่ปอดโดย septic emboli เรียกกลุ่มอาการนี้ว่า Lemierre syndrome
4. Investigation - Septic work up; CBC, Plt, H/C และ USG with doppler ดู internal jugular vein thrombosis

ขอถามต่อนะคะ
1. เชื้อก่อโรคของ Lemierre syndrome คือเชื้ออะไร
2. จะให้การรักษาอย่างไร

เพิ่งทราบจากคุณโกเมน webmaster ว่า ถ้าใช้เครื่องหมาย apostrophe ก็จะ post ไม่ได้ค่ะ กำลังหาทางแก้ไขอยู่

   1. เกิดจากการติดเชื้อ Fusobacterium necrophorum
2. การรักษาให้ Antibiotics
- Metronidazole ร่วมกับ Clindamycin
และผู้ป่วยรายนี้มี thrombosis แล้วคิดว่าน่าจะให้ LMWH; Enoxaparin เพราะเสี่ยงต่อ bleeding เนื่องจากผู้ป่วยมี Hemoptysis การให้ Heparin อาจจะ Bleed มากขึ้นค่ะ

   คุณหมอ Nongnham ตอบถูกต้องแล้วค่ะ

1. Thrombophlebitis of the right external jugular vein (Panel A, arrows).
Doppler ultrasonography confirmed thrombosis of both the external and the internal jugular veins.

2. Computed tomographic imaging of the chest revealed bilateral pleural effusion and multiple areas of consolidation with cavitation (Panel B, arrows).


Lemierre syndrome เป็นกลุ่มอาการที่ประกอบด้วย disseminated abscesses และ thrombophlebitis ของ internal jugular vein หลังมีการติดเชื้อของ oropharynx เชื้อที่สำคัญคือ gram-negative anaerobic bacillus, Fusobacterium necrophorum.

ในผู้ป่วยรายนี้ blood culture ขึ้น Fusobacterium necrophorum 2 วันต่อมา ผู้ป่วยได้รับยา metronidazole, clindamycin และ enoxaparin อาการต่างๆ ดีขึ้นอย่างรวดเร็ว ผู้ป่วยหายดีหลังจากนั้น

มีคำถามต่อค่ะ
1. รายนี้จำเป็นต้องได้รับยาทั้ง metronidazole และ clindamycin หรือ ให้ตัวหนึ่งตัวใดเพียงพอหรือไม่
2. ถ้าจะให้ยาตัวเคียว จะเลือก metronidazole หรือ clindamycin

   1. มีการศึกษา (Michael D. Williams, JCM; 2003) พบว่ามี Antibiotic regimens ที่ใช้ในการรักษา Lemeirre syndromeในกลุ่มที่เป็น Beta-lacta agent (Amoxicillin-clavulanate, ticarcillin-clavulanate, Cephalosporins-third generation) ร่วมกับ Metronidazole (high-dose penicillin + metronidazole) หรืออาจจะใช้ Monotherapy เป็น Clindamycin x 2 to 6 weeks
สำหรับการใช้ Metronidazole พบว่าเป็น attractive treatment choice สำหรับ septic thrombi เพราะผ่านเข้า tissue ได้ดีค่ะ
2. ถ้าจะเลือกใช้ยาตัวเดียวในผู้ป่วยรายนี้คิดว่าประสิทธิภาพในการรักษาอาจจะไม่ได้ผลดีค่ะเพราะในผู้ป่วยรายนี้มีภาวะ septic thrombosis แล้วค่ะ (IJV โตขึ้นและมีการแพร่กระจายไปที่ปอดแล้วค่ะ) การให้ Metronidazole + Clindamycin น่าจะมีประโยชน์ในการรักษามากที่สุดค่ะ
