ชาย 39 ปี เป็นเบาหวาน ปวดท้องแบบบีบๆ รุนแรงมา 1 สัปดาห์




   ชายอายุ 39 ปี เป็นเบาหวาน มีอาการเบื่ออาหารมา 2 เดือน ปวดท้องแบบบีบๆ อย่างรุนแรงมา 1 สัปดาห์. ผู้ป่วยกินยาสมุนไพรที่มีสรรพคุณทำให้อายุยืนมานาน 2 ปี
On examination, pallor was present. Cardiovascular, respiratory, and abdominal examination results were within normal limits.
Oral cavity: a purple blue line was present in the gingiva ( รูป 1 และ 2).

   รูปนี้เป็นของผู้ป่วยอีกรายหนึ่งซึ่งเป็นโรคเดียวกัน แต่เห็นรอยโรคชัดกว่า

   1. Lead poisoning ค่ะ เพราะมี colicky abdominal pain, Burton line (purple blue line ที่ gingiva) และมีซีดร่วมกับพบ basophilic stippling.
1. Diagnostic plan: ส่งเลือดตรวจหาระดับตะกั่วในเลือด
Therapeutic plan: ผู้ป่วยน่าจะได้รับตะกั่วจากยาอายุวัฒนะ ให้หยุดกินยานั้นและนำยามาวิเคราะห์
การรักษา lead poisoning คือการให้ penicillamine จนกระทั่งหายปวดท้อง และติดตามระดับตะกั่วในเลือด เมื่อระดับตะกั่วในเลือดลดลง Burton line ก็จะหายไป

peripheral blood smear : microcytic hypochromic anemia with basophilic stippling ( รูป 4).
Stool examination, upper gastrointestinal endoscopy, ultrasound scan of abdomen, hemoglobin electrophoresis, serum iron, serum ferritin, และ total iron-binding capacity levels ได้ผลปกติ.
Urine porphobilinogen และ ketone bodies ได้ผลลบ.
Fasting blood glucose 70 mg/dL และ postprandial blood glucose 250 mg/dL.

Blood lead level estimation โดยวิธี atomic absorption = 77.17 μg/dL (reference range < 10 μg/dL).

   การรักษาเริ่มต้นด้วย D-penicillamine 250 mg วันละ 3 ครั้ง. หลังเริ่มรักษาได้ 1 เดือน ผู้ป่วยมีอาการดีขึ้น อยากกินอาหาร และหายปวดท้อง. ผู้ป่วยกิน D-penicillamine ต่อจนกระทั่ง blood lead levels ต่ำกว่า 10 μg/dL ซึ่งใช้เวลานาน 6 เดือนตั้งแต่เริ่มให้ยา. การวิเคราะห์ทางเคมีเพื่อหาระดับตะกั่วในยาอายุรเวทของผู้ป่วยได้ผลว่ามีตะกั่ว 16,307.27 ± 447 μg ต่อแคปซูล.
