A 57-YOM had diffuse skin infiltration & sensory loss in left hand for 7 yrs.

PE: His face had multiple nodular lesions that coalesced into plaques, especially on the forehead, ears, nose, and lips (รูป A). He had weakness in the muscles on the left side of his face and loss of eyebrow hair and eyelashes. The ulnar and tibial posterior nerves were enlarged.

เพราะมี skin involvement และ nerve involvement
2. ทำ skin biopsy ส่ง patho
การรักษา: Multidrug regimens
Dapsone 100 mg/d + rifampicin 600 mg/d + clofazimine 50 mg/d นาน 24 เดือน

การวินิจฉัยคือ Lepromatous Leprosy ลักษระหน้าของผู้ป่วยมีชื่อเรียกว่าLeonine Facies

management:
- skin smear stained according to the Ziehl–Neelsen method revealed acid-fast bacilli in clumps.
- ได้ทำ screening for IgM antibodies to phenolic glycolipid I ซึ่งจำเพาะต่อ Mycobacterium leprae โดยใช้ enzyme-linked immunosorbent assay. การได้ผลบวก titer สูง เข้าได้กับการวินิจฉัย lepromatous leprosy.

lepromatous leprosy เป็น anergic form ของ leprosy ซึ่งมีลักษณะของความผิดปกติที่เกิดจากการตอบสนองของภูมิต้านทานที่ไม่เพียงพอ. Nerve damage เกิดขึ้นช้าๆ และสัมพันธ์กับการมีเชื้อจำนวนมากอยู่ในเส้นประสาท.

ผู้ป่วยได้รับการรักษาด้วยยาต้านจุลชีพหลายคัว ร่วมกับ prednisone นาน 5 เดือน.
หลังจากได้รับยามา 9 เดือน รอยโรคที่เกิดจาก skin infiltration และอาการอ่อนแรงของหนังตาซ้ายหายไป (รูป B).

ระยะเวลาสำหรับการรักษา standard multibacillary leprosy คือ 12 เดือน แต่สำหรับ anergic polar forms ดังที่พบในผู้ป่วยรายนี้ ต้องการการรักษานานเป็น 24 เดือนเพื่อให้หายขาด.
