A 46-YOW had fever, back pain & a slowly growing Rt paravertebral mass for 8-mos.




   หญิงอายุ 46 ปีมาพบแพทย์เนื่องจากมีไข้ ปวดเอวข้างขวา และมีก้อนซึ่งค่อยๆ โตขึ้นอยู่ติดกับด้านขวาของกระดูกสันหลังมานาน 8 เดือน. ผู้ป่วยกินเนยที่ทำจากนมแพะซึ่งไม่ได้ฆ่าเชื้อ (unpasteurized) เป็นประจำ.

Physical examination revealed a fluctuant right paravertebral mass and marked pain on palpation over the lumbar spine at the L3-L4 level.

ในรูป 1 คือ computed tomography (CT) scan of the spine พบว่ามี erosions of the end plates of L3 and L4 and the antero-superior angle of L3, narrowing of the intervertebral space between L3 and L4, and new bone formation.

   รูป 2 คือ magnetic resonance imaging (MRI) of the spine พบว่ามี widening of the right iliopsoas muscle ด้วย.

1. เชื้อก่อโรคในผู้ป่วยรายนี้น่าจะเป็นเชื้ออะไร
2. จะ manage อย่างไร

   ผู้ป่วยมีปัญหา
1. มีไข้ ปวดเอวข้างขวา และมี right paravertebral mass มานาน 8 เดือน.
2. กินเนยที่ทำจากนมแพะซึ่งไม่ได้ฆ่าเชื้อ (unpasteurized) เป็นประจำ.
3. CT+MRI พบ bone destruction ที่ L3 และ L4

การวินิจฉัย
vertebral osteomyelitis with paravertebral abscess
คิดถึงเชื้อก่อโรค 2 ตัวค่ะ คือ
1. Brucella spp. เพราะมีประวัติกินเนยที่ทำจากนมแพะซึ่งไม่ได้ฆ่าเชื้อ (unpasteurized) เป็นประจำ.
2. Mycobacterium tuberculosis

Plan: เจาะหนอง จาก fluctuant right paravertebral mass ย้อมสีแกรม และ acid fast และส่งหนองเพาะเชื้อสำหรับ aerobic bacteria และ mycobacteria

   คุณหมอ Daisy1 ตอบถูกแล้วค่ะ

Tube agglutination test for brucellosis ได้ผลบวกที่ titer 1/640, และ CT-guided percutaneous drainage ของ paravertebral mass ได้หนองซึ่งเพาะเชื้อขึ้น Brucella melitensis.

ผู้ป่วยได้รับการรักษาด้วย doxycycline, gentamicin และ rifampicin และตอบสนองต่อการรักษาดี.

   Bacterial vertebral osteomyelitis อาจเกิดจาก pyogenic bacteria, Mycobacterium tuberculosis, และ B. melitensis.

Tuberculosis เป็นที่ thoracic บ่อยที่สุด; พบ paravertebral masses ได้บ่อย, แต่พบ new bone formation ได้น้อย.

Pyogenic vertebral osteomyelitis, ที่สำคัญเกิดจาก Staphylococcus aureus, แสดงอาการโรคและการตรวจพบทาง radiograph เหมือน brucellosis.

ผู้ป่วยที่เป็น brucellosis จาก B. melitensis เกิดจากการกินอาหารที่เป็นผลิตภัณฑ์ของนมที่ปนเปื้อนเชื้อ. Vertebral osteomyelitis และiliopsoas abscess เป็นภาวะแทรกซ้อนที่ทราบกันดีของ brucellosis ที่ไม่ได้รับการรักษา.
การรักษาด้วยยาปฏิชีวนะ 3 ตัวร่วมกันทำให้ผลการรักษาดีขึ้น.
