A 49-YOM had painful rash with burning sensation at leg for 4-days




   ชายอายุ 49 ปีมารพ.เนื่องจากมีผื่นที่หน้าแข้งข้างขวามา 4 วัน ผู้ป่วยรู้สึกแสบร้อนที่ผื่น. ผู้ป่วยได้รับการผ่าตัด orthotopic liver transplantation สำหรับโรคติดเชื้อ hepatitis C เมื่อ 20 ปีก่อน.

On examination, hemorrhagic, papulovesicular lesions (รูป 1), 0.5–1.5 cm in size with central confluence and vesicle formation, were noted.

   1.จากระประวัติ :"มีผื่นที่หน้าแข้งข้างขวามา 4 วัน ผู้ป่วยรู้สึกแสบร้อนที่ผื่น" แสดงว่าจุดที่เกิดพยาธิสภาพควรจะเป็นผิวหนังหรือเส้นเลืดบริเวณผิวหนังไม่ควรเป็นที่เส้นประสาทบริเวณผิวหนัง
2.จากลักษณะผื่น :ผื่นมีลักษณะเป็นถุงน้ำ "หลังคาของผื่นไม่ตึง" แสดงว่าพยาธิสภาพอยู่ตึ้นใน Epidermis แน่นอน ผื่น "Vesicle" แสดงว่าจะต้องมี ของเหลว อยู่ภายในซึ่งอาจจะเป็น เลือด น้ำ หรือ หนอง แต่ในที่นี้สีแดงเพราะฉะนั้นควรจะเป็นเลือดที่อยู่ข้างใน เมื่อเป็น "เลือดอยู่ข้างใน" ถามว่าเลือดเหล่านั้นมาจากไหน ก็คงจะต้องมาจากเลื้นเลือดเล็กๆที่บริเวณผิวหนังเป็นแน่ "เกิดอะไรกับเส้นเลือด?" ผมคิดว่า Vasculitis เป็นคำอธิบายที่ดีที่สุด "Vasculitisจากอะไร?" อาจจะเป็นจาก Immume complex หรือ Infection แต่ควรจะมาจาก infection เพราะ เป็น Local Vasculitis เพราะถ้าเป็น Immume complex ควรเป็น Systemic Vasculitis "Infection อะไร?"สรุปจากที่กล่าวมาทั้งหมด "Local Epidermal hemorrhagic papulovesicular lesions from vasculitis" ตัวที่ทำแบบนี้ได้คือ Virus ในตระกูล Herpesvirus ซึ่งก็มีสมาชิกในตระกูลนี้หลายตัว "ตัวไหน?" จากลักษณะ distribution ที่เป็น Linear ก็ต้องคิดถึง varicella zoster virus ก่อนตัวอื่นๆครับ "จะ manage อย่างไร?" ผมคิดว่าก็ควรเจาะน้ำในตุ่มไปตรวจ และจะต้องทำ skin biopsy ด้วยเพื่อดูว่า vasculitis จริงหรือป่าวครับ

   Diagnosis: Varicella zoster virus–associated leukocytoclastic vasculitis

punch biopsy specimen (รูป 2) : an intraepidermal vesicle with profound acantholysis in the epidermis. A multinucleated giant cell (arrowhead) and a balloon cell with an intranuclear viral inclusion body (arrow) are visible inside the vesicle. Examination of the dermis showed leukocytoclastic vasculitis, with fibrinoid degeneration of the vessels and infiltration with inflammatory cells, including neutrophils with leukocytoclasia (neutrophilic nuclear debris) and extravasation of
the erythrocytes.the erythrocytes.

การทำ viral culture ของ biopsy specimen ขึ้น varicella zoster virus. ผู้ป่วยได้รับการรักษาด้วย IV acyclovir ทำให้รอยโรคหายสนิท.

   เชื้อ herpesviruses, เช่น varicella zoster virus, herpes simplex virus, cytomegalovirus, และ human herpesvirus-6, สัมพันธ์กับการเกิด leukocytoclastic vasculitis. มีรายงานว่า leukocytoclastic vasculitis เกิดขึ้นประมาณ 30% ในผู้ป่วยที่มี varicella zoster virus infection. ลักษณะที่มีเลือดออกในรอยโรคของผู้ป่วยรายนี้น่าจะเกิดจาก vasculitic component เป็นสำคัญ.
