A 40-YOW with abdominal pain, diarrhea +hypotension 19 day after discharge


   หญิงอายุ 40 ปีมารพ.เนื่องจากปวดท้องแบบบีบๆ คลื่นไส้ อาเจียนและถ่ายอุจจาระเป็นน้ำ. CBC มี leukocytosis (12,900 white cells/mL). ผู้ป่วยได้เข้ารับการรักษาในรพ.ด้วยการวินิจฉัยว่า infectious colitis และได้รับ ciprofloxacin นาน 14 วัน, โดยผู้ป่วยมีอาการดีขึ้นมาก.

9 วันหลังออกจากรพ. ผู้ป่วยมาที่แผนกฉุกเฉินอีกครั้งเพราะมีปวดท้อง คลื่นไส้ อาเจียน ถ่ายอุจจาระเหลว และมีความดันโลหิตต่ำ.

2. จะ manage อย่างไร

   S:1.หญิงอายุ 40 ปี ปวดท้องแบบบีบๆ คลื่นไส้ อาเจียนและถ่ายอุจจาระเป็นน้ำและความดันโลหิตต่ำ
2.เคยได้รับยาปฏิชีวนะ ciprofloxacin นาน 14 วัน
O:CBC มี leukocytosis (12,900 white cells/mL)
A:Psuedomembranous Colitis
P:1.Resuscitation for hypovolumia by NSS
2.Metronidazole (500mg)PO tid for 10-14 day หรืออาจจะให้ Vancomycin 125-500 mg PO qid ร่วมโดยถ้ามีอาการของ SIRS
3.Stool ELISA หา Clostridium toxin A+/-B

ผู้ป่วยได้รับการทำ contrast-enhanced computed tomographic scan of the abdomen ซึ่งพบว่ามี marked bowel-wall thickening (รูป A, arrows) throughout the sigmoid colon and descending colon.

   Colonoscopy (รูป B) revealed multiple discrete, yellowish, polypoid lesions and a friable, hyperemic mucosa.

   Histopathological examination of the biopsy specimens revealed a neutrophilic infiltrate in the lamina propria and mucopurulent exudates erupting through the denuded surface epithelium, findings that confirmed a diagnosis of pseudomembranous colitis (รูป C, hematoxylin and eosin).

   ผลการทดสอบหา Clostridium difficile toxin ซึ่งทำที่ 6 วันหลังการเข้ารับการรักษาในรพ.ครั้งที่ 2 ได้ผลลบ. ผู้ป่วยเริ่มได้รับการรักษาด้วยยาต้านจุลชีพและได้กลับบ้านหลังจาก colitis หาย.
