A 71-YOM had severe generalalized pruritus for 9 mo.

ชายอายุ 71 ปีได้ถูกส่งตัวมาพบแพทย์ที่คลินิโรคภูมิแพ้เนื่องจากมีอาการคันทั้งตัวมานาน 9 เดือน ผู้ป่วยได้รับการรักษาโดยแพทย์หลายคนมาก่อน ได้รับ antihistamines และ oral corticosteroids โดยไม่ดีขึ้น.
3 ปีก่อน ผู้ป่วยมีอาการชาที่แขนและขาโดยค่อยๆ เป็นมากขึ้น. จากการตรวจด้วย electromyography และ nerve conduction velocity testing แพทย์ให้การวินิจฉัย chronic inflammatory demyelinating polyneuropathy และให้การรักษาด้วย oral corticosteroids แต่อาการไม่ดีขึ้น. ผู้ป่วยมีโรคเดิมคือ asthma, chronic rhinosinusitis, hyperlipidemia, type 2 diabetes mellitus และ benign prostatic hyperplasia.
สภาพแวดล้อมที่บ้าน ไม่มีสัตว์เลี้ยงและไม่ชื้น. ผู้ป่วยไม่สูบบุหรี่ เขาทำงานเป็นผู้คุมนักโทษ. ยาที่ใช้อยู่ คือ albuterol, aspirin, atorvastatin, hydrochlorothiazide, ipratropium, metformin, salmeterol/fluticasone, tamsulosin และ valsartan.

Assessment
PE: He had diffuse skin excoriations but no rash. Prolonged expirations, unsteady gait, and sensorimotor deficits in both lower extremities also were identified.
Peak expiratory flow rate ของผู้ป่วยลดลง (330 L/min; predicted, 477 L/min).
Percutaneous skin testing ด้วย 14 common inhalant allergens รวมถึง mold spores หลายชนิด ได้ผลลบ.
Pulmonary function testing: decreased forced expiratory volume in 1 second (FEV1) and decreased forced vital capacity (FVC), normal FEV1/FVC ratio, and no significant bronchodilator response.
Chest x-ray: old inflammatory disease but no active disease.
CT chest, performed without contrast, demonstrated bilateral lung nodules with diffuse ground-glass opacities.
Complete blood count: marked leukocytosis (18.4 x 103 cells/mm3) with 59% eosinophils; the absolute eosinophil count was 8100.

problem list

5. CT chest, performed without contrast, demonstrated bilateral lung nodules with diffuse ground-glass opacities.
6. Hypereosinopphilia: CBC: wbc 18,400cells/mm3) with 59% eosinophils; the absolute eosinophil count was 8100.

จากการที่เขามีโรคเดิม ทำให้ต้องได้รับการรักษาด้วย steroid และการมี hypereosinophilia สาเหตุของการคันจึงนึกถึง strongyloidiasis มากที่สุดค่ะ เพราะ steroid ทำให้ rhabditiform larva กลายเป็น filariform larva มากขึ้น เกิด autoinfection เพิ่มขึ้น

ขอส่ง stool exam for S. stercoralis larva ค่ะ ส่งสัก 3 วันติดต่อกัน

Filariform larva ของ Strongyloides stercoralis ค่ะ แสดงว่าผู้ป่วยมี autoinfection มาก เป้น hyperinfection syndrome หรือ disseminated strongyloidiasis

การรักษา
1. Uncomplicated strongyloidiasis ยาที่เลือกใช้รักษา strongyloidiais คือ ivermectin และ albendazole เป็น alternative

Ivermectin — ให้ยาขนาด 200 mcg/kg dose ของ ivermectin 2 ครั้งโดยให้ติดกัน 2 วันหรือห่างกัน 2 สัปดาห์

Albendazole — Albendazole (400 mg PO วันละ 2 ครั้งนาน 3-7 วัน. การศึกษาในคนไทย 42 รายที่มี chronic strongyloidiasis พบว่าการรักษาด้วยsingle dose of ivermectin มี cure rate 90 % เทียบกับ 50 % เมื่อให้ albendazole 800 mg/day นาน 7 วัน

2. Disseminated disease/hyperinfection syndrome
ในผู้ป่วย immunocompromised patients ถ้าทำได้ ให้ลด immunosuppressive therapy ร่วมกับการให้ anthelminthic therapy คือให้ ivermectin ร่วมกับ albendazole จนผู้ป่วยตอบสนองต่อการรักษา คืออาการหายไปและตรวจอุจจาระไม่พบพยาธิอย่างน้อย 2 สัปดาห์.

ขอตอบโรคอื่น ที่ไม่ใช่ โรคติดเชื้อได้ไหมครับ ดูจากประวัติ อาจารย์เน้นตรง asthma , chronic rhinosinusintis , pulmonary infiltrate , CIDP , hypereosinophilia
ซึ่งเข้าได้กับ Churg- Strauss syndrome จาก American college of Rheumatology 1990 :
- asthma
- Eo >10%
- Pulmonary infiltrates
- paranasal sinus abnormality
- Extravascular eosinophila on biopsy
ถ้ามีอย่างน้อย 4 ใน 6 ข้อ sen.83% and spec 99.7%
ใน case นี้น่าจะทำ skin biopsy ดูนะครับ

ขอขอบคุณคุณหมอ nimbus ที่ให้ความเห็นมาว่าควรนึกถึงโรคอื่นๆ ด้วย ดิฉันเห็นด้วยกับคุณหมอค่ะ

Laboratory analyses: serum chemistries; liver function tests; testing for antinuclear antibodies, antineutrophilic cytoplasmic antibodies, และ human immunodeficiency virus antibodies; sedimentation rate; urine และ serum protein electrophoresis; และ serum IgA, IgG, และ IgM levels อยู่ในเกณฑ์ปกติทั้งหมด.
Total serum IgE สูงมากด้วย (9562 IU/mL; normal, <180 IU/mL).

จากการ review medical records ของผู้ป่วย พบว่า eosinophil count เริ่มสูงขึ้นเมื่อ 5 ปีก่อนโดยก่อนเริ่ม corticosteroid therapy, eosinophil count ประมาณ 20% อยู่ 3 ครั้งและหลังจากเริ่มได้รับ corticosteroid อย่างต่อเนื่อง eosinophil level สูงถึง 47% และสูงอยู่ประมาณนี้หลายครั้งมากใน 30 เดือนต่อมา.

เมื่อ 2 ปีก่อน ผู้ป่วยได้รับการตรวจอุจจาระหาพยาธิและไข่พยาธิหลายครั้ง เนื่องจากมี eosinophilia แต่ได้ผลลบ. เมื่อ 15 เดือนก่อน ได้รับการตรวจ colonoscopy พบว่ามี severe eosinophilic bowel inflammation แต่ไม่พบพยาธิ. ยังไม่รู้สาเหตุของ eosinophilic colitis.

Parasite serologies ที่ทำเมื่อผู้ป่วยมาอยู่โรงพยาบาลครั้งนี้พบว่า ได้ผล positive Strongyloides antibodies โดย enzyme-linked immunosorbent assay (observed level, 77 units; positive, >8 units), positive serum antifilarial IgG1, และ positive serum total antifilarial antibody (observed level, 19.7 mg/mL; positive, >14.0 mg/mL).
Serum antifilarial IgG4, stool ova และ parasites, และ blood microfilariae levels ได้ผลลบ.

การรักษา
ได้ให้การรักษาด้วย ivermectin 200 mcg/kg/dayX 2-day course สำหรับ presumed S. stercoralis infection. ผู้ป่วยตอบสนองต่อการรักษาดีมาก. อาการคันหายไปอย่างรวดเร็ว และอาการทางปอดก้ดีขึ้นอย่างมาก.
หนึ่งเดือนหลังการรักษา eosinophil count ลดลงเป็น 5%, และอยู่ในระดับนี้ในช่วง 9 เดือนต่อมา. 8 เดือนหลังการรักษา serum IgE ลดลงเป็น 1718 IU/mL, และที่ 12 เดือนก็ลดเป็น 947 IU/mL. pulmonary function testing หลังการรักษาแสดงถึง marked improvements in obstruction, with increased FEV1 and FVC. อาการอย่างเดียวที่ไม่ดีขึ้นที่ 12 เดือนหลังการรักษาด้วย ivermectin คือ polyneuropathy.

การวินิจฉัย S. stercoralis infection ที่แน่นอนคือการตรวจพบ larvae ในอุจจาระ แต่ในผู้ป่วยส่วนใหญ่มี intestinal worm load ต่ำและ larva ถูกขับออกมาน้อย. การตรวจอุจจาระเพียงครั้งเดียวพบ S. stercoralis ได้เพียง 30% ของ cases. การตรวจอุจจาระซ้ำเพิ่ม sensitivity: มีรายงานว่าการตรวจอุจจาระติดต่อกัน 7 ครั้งทำให้ได้ diagnostic sensitivity ใกล้เคียง 100%.

Strongyloides serology ให้ผล 95% sensitive และ 29% specific; negative และ positive predictive values 95% และ 30%, ตามลำดับ. แต่ผลที่ออกมาเป็นบวกก้ต้องแปลอย่างระมัดระวัง เนื่องจาก serologies นี้มี cross-react กับการติดเชื้อปรสิตอื่นๆ คือ filariasis, schistosomiasis, และ Ascaris lumbricoides.
การติดเชื้อปรสิต ซึ่งรวมถึง S. stercoralis, สัมพันธ์กับการเพิ่มของระดับในซีรัมของ antigen-specific IgE และ helminth-specific IgE สามารถคงอยู่เหลายปีหลังการรักษาโดยไม่ได้รับพยาธิเข้าไปใหม่.

Chest radiographic findings ที่สัมพันธ์กับ S. stercoralis ไม่มีลักษณะจำเพาะ โดยอาจพบ pleural effusions หรือ interstitial, alveolar, segmental, หรือ lobar pulmonary infiltrates.

ในผู้ป่วยรายนี้ ไม่พบ S. stercoralis จากการตรวจด้วยกล้องจุลทัศน์. การมี positive antifilarial antibodies น่าจะเป็นจาก cross-reactivity.
