A 45 YOF with fever and pain at right upper abdomen




   From Rickes S., Ocran K. N Engl J Med 2002;346:e4.

A 45-year-old woman was hospitalized with a seven-day history of fever and pain in the right upper abdomen.
Ultrasonography (Panel A) and CT abdomen (Panel B) were performed.
1. What are the abnormalities?
2. What are the most likely diagnosis?

   1. USG and CT abdomen showed hypoechoic lesion with irregular walled in the liver.
2. Amoebic liver abscess

   กลับไปดู USG และ CT อีกรอบ ขอตอบใหม่นะคะ
1. Finding
- distended gallbladder with a slightly thickened wall
- calcified lesion (stone) in gallbladder
may be impacted obstructing stone in the neck of the gallbladder
2. Acute calculous cholecystitis

   อ.ศรัญญูตอบถูกต้องแล้วค่ะ

Ultrasonography (Panel A) showed gallstones (large arrows), including one in the cystic duct, leading to an enlargement of the gallbladder (diameter about 5 cm) and calcification of the wall (small arrows).

Computed tomography (Panel B) confirmed the presence of calcification of the gallbladder wall, or porcelain gallbladder (small arrow), and gallstones (large arrow).

คำถาม

ผู้ป่วยมีไข้และปวดท้องมา 7 วัน จะให้การรักษาผู้ป่วยอย่างไร ต้องให้ยาอะไร

   วินิจฉัยว่า เป็น Acute cholecystitis with CBD obstruction ไม่สามารถตัดภาวะ ascending cholagitis ได้
medical treatment : ATB covering enterobacteriacae with/without ATB covering anaerobic bacteria ( Ceftiaxone 2 gm IV q 24 hrs +Ampicillin 2 gm IV q 6 hrs +/- Metronidazole 500 mg IV q 8 hrs )
Intervention treatment
-Cholecystectomy
-intra operative ERCP with stone removal

The patient declined cholecystectomy, which is the usual treatment, and was treated with antibiotics. Two weeks later, the fever and pain had resolved.
A large, solitary calcification in the right upper abdomen may indicate disease of the gallbladder, adrenal glands, kidneys, lungs, or chest wall. With respect to the diagnosis of porcelain gallbladder, ultrasonography is as sensitive as abdominal radiography and computed tomography. Patients with a calcified gallbladder have an increased risk of gallbladder cancer.

รายนี้ไม่มีรายละเอียดของอาการของผู้ป่วยว่าเขา severe ขนาดไหน มี sepsis หรือไม่ คิดว่าเขาไม่น่าจะมี acute cholangitis เพราะไม่มี CBD stone

คำถาม จะให้ ceftriaxone อย่างเดียวได้หรือไม่ จำเป็นต้องให้ ampicillin และ metronidazole ด้วยหรือ
