A 17-YOM with coronary aneurysm


   ชายอายุ 17 ปี แพทย์นัดมาตรวจ Virtual coronary endoscopy ผู้ป่วยมีประวัติการเจ็บป่วยดังนี้
-เมื่ออายุ 9 ปี ผู้ป่วยมี conjunctival injection, strawberry tongue, และมีไข้ต่ำๆ เป็นอยู่นาน 2 สัปดาห์แล้วหายไปเองโดย ผู้ป่วยได้รับเพียง supportive care.
-เมื่ออายุ 10 ปี เขามีอาการเจ็บแน่นหน้าอกร่วมกับมีเหงื่อออกมากเป็นครั้งคราว coronary angiography: an aneurysm of the left anterior descending coronary artery.

ถาม: Coronary aneurysm ในผู้ป่วยรายนี้เกิดจากโรคใด

   คุณหมอ cadet ตอบถูกต้องแล้วค่ะ

ผู้ป่วยได้รับการรักษาด้วย Antiplateletและติดตามด้วย cardiac computed tomography.

Virtual coronary endoscopy (ทำเมื่อผู้ป่วยอายุ 17 ปี) revealed an aneurysm with a rough, corrugated surface. The diagrams in the left upper and lower corners of the figure represent the angle of view of the endoscope and the comparatively smooth surface of the normal intima, respectively. The right upper and lower diagrams represent coronary angiographic and 3-dimensional images of the aneurysm, respectively.

   Coronary aneurysm เป็น major complication อย่างหนึ่งของ Kawasaki disease ซึ่งอาจทำให้เกิด myocardial infarction ในผู้ป่วยอายุน้อยหรืออาจเกิดหลังจากนั้น. ผู้ป่วยโรคนี้ต้องการการติดตามดูแลตลอดชีวิตร่วมกับการรักษาด้วย anticoagulant และ antiplatelet.
