Examination revealed an ulcer measuring 1 by 2 cm on the left side of his neck (Panel A), with an underlying mass measuring 2 by 2 cm. The chest x-ray film was normal. Computed tomography revealed multiple enlarged lymph nodes on both sides of the neck with central necrosis (Panel B, asterisks). 1. What is the most likely diagnosis ? 1. What is the most likely diagnosis ? ต้องนึกถึง Tuberculosis มากที่สุด differential diagnoses: 1. non-tuberculous mycobacterial infection 2. melioidosis 3. nocardiosis 4. fungal infection ต่างๆ เช่น cryptococcosis, histoplasmosis 2. Which investigations you will do? CD4 count = 104 cells/ cumm. Scrofula (tuberculous lymphadenitis at cervical region) presents as chronic, nontender lymphadenopathy, which may fistulize and drain cutaneously.
