A18-YOW had left upper quadrant abdominal pain for 5 mo.

หญิงอายุ 18 ปีซึ่งแข็งแรงดีมาก่อนมาพบแพทย์เนื่องจากปวดท้องด้านซ้ายบน ท้องโตขึ้น อาเจียนหลังกินอาหาร และน้ำหนักลด 18 กก.
Physical examination revealed a firm, tender, epigastric mass but was otherwise unremarkable.

1. ในรูป A คือ CT abdomen ของผู้ป่วย พบความผิดปกติอะไร
2. การวินิจฉัยน่าจะเป็นอะไร
3. จะ manage อย่างไร

Computed tomography showed a large gastric mass extending from the fundus to the antrum (รูป A, arrow), with no indication of obstruction of the gastric outlet.
Esophagogastroduodenoscopy revealed a large mass occluding nearly the entire stomach, without extension into the duodenum (รูป B).

อาจารย์เฉลยให้แล้วค่ะ ภาพที่เห็น จาก CT abdomen ลักษณะ round mixed iso and hypodensity mass inside gastric lumen และมี air ตรงขอบๆ เหมือน gastric content --trichobezoar จากการกลืนเส้นผมพบในผู้หญิงอายุน้อย สัมพันธ์กับภาวะ trichotillomania,iron deficiency and pica syndrome ถ้ามี content move down to duodenum or cause gut obstruction เรียกว่า Rapanzel syndrome
-treatment with remove FB if cannot remove by scope must be explore
-treatment of anemia
-consult psychologist

ถามประวัติผู้ป่วย ผู้ป่วยยอมรับว่าเธอมีนิสัยชอบกินผมของตัวเองมานานหลายปี ภาวะนี้มีชื่อเรียกว่า trichophagia. เนื่องจาก trichobezoar นี้มีขนาดใหญ่มาก (37.5 x 17.5 x 17.5 ซม.), จึงไม่สามารถเอาออกด้วยการทำ endoscopy. ได้พยายามทำ laparoscopic removal แต่ก็เอาก้อนออกไม่ได้ สุดท้ายต้องผ่าตัดเอาก้อน trichobezoar ซึ่งมีน้ำหนัก 4.5 กก.ออก (รูป C). ผู้ป่วยกินอาหารทั่วไปได้ในวันที่ 5 หลังผ่าตัดและได้กลับบ้าน โดยมีนัดมาพบศัลยแพทย์และจิตแพทย์.
1 ปีหลังการผ่าตัด ผู้ป่วยไม่ปวดท้องและไม่อาเจียน. น้ำหนักเพิ่มขึ้น 9 กก.และผู้ป่วยหยุดกินผมตัวเองแล้ว.
