A 67-YOW had a 6-month history of dyspnea and dysphagia.




   หญิงอายุ 67 ปีมารพ.เนื่องจากมีอาการเหนื่อยและกลืนลำบากมา 6 เดือน.
Physical examination revealed a small cervical goiter, but the lower poles of the thyroid were not palpable.

1. ในรูป A คือ Chest radiography ของผู้ป่วย พบความผิดปกติอะไรบ้าง
2. การวินิจฉัยน่าจะเป็นอะไร
3. จะ manage อย่างไร

   1. จาก CXR tracea ถูกดันมาข้างซ้ายโดยก้อน (ซึ่งจากประวัติและการตรวจร่างกาย เป็น substernal goiter ได้)
2. Substernal goiter
3. ส่งตรวจ Thyroid function test ดูว่ามีhyperthyroid หรือไม่ และส่ง US หรือCT scan เพื่อดูขนาดของก้อนและ involve organ อื่นใดบ้าง
ปรึกษาศัลยกรรมด้วยค่ะ

   คุณหมอ lara ตอบถูกต้องแล้วค่ะ

-Chest radiography (รูป A) showed tracheal deviation (arrows).
-serum thyrotropin และ free thyroxine levels ปกติ.
-Ultrasonography of the neck revealed a large goiter with the right lobe extending into the anterior superior mediastinum; the lower part of the mass was not detectable because of the sternum.

   Cervical and mediastinal computed tomography (Panels B and C, respectively) showed a large goiter (8 by 6 by 10 cm) extending from the laryngeal cartilage into the mediastinum, displacing vascular structures (black arrows) and causing both compression and deviation of the trachea (white arrows).

ผู้ป่วยได้รับการผ่าตัด total thyroidectomy และ ผลการตรวจทาง histology เป็น multinodular goiter. อาการของผู้ป่วยหายไปหมดหลังการผ่าตัด.
