A 10-YOG had episodic wheezing, dyspnea & exercise intolerance for 9 mo.




   เด็กหญิงอายุ10 ปีซึ่งเดิมแข็งแรงดี มารพ.เนื่องจากมีเสียงหายใจดังวี๊ดและเหนื่อยเป็นครั้งคราว และออกแรงได้น้อยลงซึ่งเป็นมากขึ้น ทั้งหมดเป็นมานาน 9 เดือน.
On physical examination, she had moderate bilateral expiratory wheezing.
ผู้ป่วยได้รับการรักษาแบบ asthma โดยได้รับ inhaled bronchodilators, inhaled corticosteroids, montelukast และ oral corticosteroids หลาย course. แม้ผู้ป่วยได้บอกว่าอาการของเธอดีขึ้น แต่ไม่มี demonstrable objective improvement.
เพื่อประเมินอาการของผู้ป่วย จึงได้ส่งทำ spirometry

1. จาก spirometry (รูป A) แปลผลว่าอย่างไร
2. จะ manage อย่างไรต่อไป

Plan: ควรทำ bronchoscopy ว่ามีการอุดตันของหลอดลมหรือไม่และเกิดจากอะไร

   Spirometry (รูป A) revealed severe expiratory obstruction.
ผู้ป่วยได้รับการทำ flexible fiberoptic bronchoscopy, ซึ่งพบว่ามี nearly complete (>90%) occlusion of the distal trachea by a vascular mass.
Computed tomographic angiography showed that the mass did not extend beyond the trachea (รูป B, arrow).

ก้อนนี้ได้ถูกตัดออกด้วยการส่องกล้องและใช้ potassium titanyl phosphate laser. Pathological analysis: an inflammatory pseudotumor, a benign tumor composed of a proliferation of inflammatory cells. เนื้องอกชนิดนี้พบน้อยว่ามี invasive และหายได้จาก local excision.

   1 เดือนหลังการผ่าตัด ผู้ป่วยไม่มีอาการผิดปกติใดๆ และ spirometry ได้ผลปกติ (รูป C).
Pulmonary pseudotumors เป็นสาเหตุที่พบน้อยของ wheezing. อย่างไรก็ตาม การไม่ตอบสนองต่อ first-line therapies ทำให้ต้องมี investigation ต่อไปซึ่งรวมถึง spirographic analysis.

คุณหมอ lara ตอบถูกแล้ว เก่งมากค่ะ
