A 54-YOW had abnormal acoustic sensations, aphasia & visual-field disturbances.




   หญิงอายุ 54 ปีมี progressive abnormal acoustic sensations, aphasia, และ visual-field disturbances. ไม่มีประวัติ head trauma หรือการติดเชื้อ (เช่น otitis media) ก่อนหน้านี้.

1. ในรูป A คือ cranial radiograph พบความผิดปกติอะไรบ้าง
2. สาเหตุของความผิดปกตินี้น่าจะเกิดจากอะไร
2. จะ manage อย่างไรต่อไป

   1. ในรูป A คือ cranial radiograph พบว่ามี air อยู่ที่ left temporal area ไม่เห็นร่องรอย fracture ของ bone
2. สาเหตุของ pneumocephalusเกิดขึ้นได้จาก head/face trauma, postneurosurgical procedure หรือการติดเชื้อของหูค่ะ
2. management: CT scan of brain และปรึกษา neurosurgeon ค่ะ

   Cranial radiograph (รูป A) revealed air in the left temporal region without evidence of a fracture (arrow).

Computed tomographic scan of the head showed a large amount of air in the left temporal lobe; the involved area was approximately 4 cm by 3 cm by 5 cm (รูป B, arrow). ความผิดปกตินี้ทำให้เกิด significant compression ของ left lateral ventricle และ sulci และ gyri ที่อยู่ข้างๆ ; มี midline shift, ร่วมกับมีอาการแสดงของ uncal herniation และ slight brain-stem rotation เล็กน้อย.

ผู้ป่วยได้รับการผ่าตัด left temporal craniotomy ซึ่งพบว่ามี defect ใน anterolateral surface ของ mastoid และได้รับการซ่อมแซม. ไม่พบ specific underlying pathologic lesion เช่น เนื้องอกหรือการติดเชื้อ.

   Pneumocephalus สามารถเกิดขึ้นได้หลัง neurosurgical procedures, trauma ของศีรษะและใบหน้า , หรือการติดเชื้อของหู และยังสามารถเกิดขึ้นได้เอง ดังเช่นในผู้ป่วยรายนี้. แม้ว่าอากาศปริมาณน้อยมักจะถูกดูดซึมกลับไปโดยไม่มีภาวะแทรกซ้อนใดๆ แต่ในผู้ป่วยที่มี air entrapment เพิ่มมากขึ้น, ก็อาจเกิดอาการทางคลินิกได้ดังเช่นผู้ป่วยรายนี้.
หลัง surgical decompression อาการของผู้ป่วยหายไปอย่างรวดเร็ว. ไม่มีภาวะแทรกซ้อนใดๆ หลังผ่าตัด และผู้ป่วยไม่มีความผิดปกติใดๆ ที่ 1 ปีต่อมา.
