A 23-YOW had premature rupture of the membranes at 29 weeks” gestation.

หญิงอายุ 23 ปี (gravida 1, para 0) ได้เข้ารับการรักษาในรพ.เมื่ออายุครรภ์ 29 1/7 สัปดาห์ เนื่องจาก preterm premature rupture of the membranes.
ถุงน้ำแตกเมื่อ 4 วันก่อน (confirmed ด้วย speculum examination). ได้ทำ continuous electronic monitoring ของ fetal heart rate ตั้งแต่แรกรับและพบว่ามี reactive tracing and occasional mild, variable decelerations.

ในรูปคือ Color Doppler ultrasonography ซึ่งพบว่ามี umbilical cord prolapsing ผ่าน cervix โดยที่ cervix ไม่ได้ markedly dilated.

ขอเฉลยเลยค่ะ

ผู้ป่วยได้รับการผ่าตัด emergency cesarean section, และได้ทารกเพศชายหนัก 1180 กรัม. Apgar scores ของทารกเท่ากับ 6 ที่ 1 นาทีและ 8 ที่ 5 นาที.

Analyses ของ umbilical-cord arterial blood gases ได้ค่า pH of 7.325 และ base excess of –2.3 mEq per liter. ทารกได้ถูกย้ายไป newborn intensive care unit และมีอาการดีตลอด.
