An 89-YOW had nonpalpable, nonpruritic hyperpigmentation of her legs




   หญิงอายุ 89 ปีมารพ.เนื่องจากมีแผลที่ไม่หายที่ขาขวา. ผู้ป่วยกิน minocycline (100 mg) วันละ 2 ครั้งเพื่อเป็น suppressive therapy สำหรับการติดเชื้อ corynebacterium ซึ่งได้เกิดขึ้นหลังการผ่าตัด orthopedic
fixation สำหรับ intertrochanteric hip fracture เมื่อ 28 เดือนก่อน.
Her physical examination was notable for a nonpalpable, nonpruritic hyperpigmentation of her legs (รูป A และ B) and arms.

   nonpalpable, nonpruritic hyperpigmentation of her legs ซึ่งเป็นสี blue-gray ในผู้ป่วยรายนี้น่าจะเกิดจากการได้รับ minocycline เป็นเวลานาน

management คือ หยุดยา minocycline ค่ะ แต่ใช้เวลาหลายเดือนกว่าจะจางไป
มีรายงานการใช้ Q-switched laser เพื่อกำจัด hyperpigmentation จะรวดเร็วและได้ผลดี

   คุณหมอ Lara ตอบถูกแล้วค่ะ

-แพทย์ได้วินิจฉัย minocycline-induced hyperpigmentation และได้เปลี่ยนยาเป็นปฏิชีวนะเป็น cefadroxil.
-มียาหลายตัวที่ทำให้เกิด hyperpigmentation, ซึ่งรวมถึง minocycline, amiodarone, zidovudine และ bleomycin.
-Minocycline-induced hyperpigmentation สามารถทำให้เกิด severely disfiguring และมีโอกาสสูงที่จะเกิดกับผู้ป่วยบางกลุ่ม เช่น ผู้ป่วยโรคpemphigus, pemphigoid, หรือ atopic dermatitis
-มีความสำคัญที่ต้อง recognize ภาวะนี้ได้เร็วและให้ยาอื่นแทน เนื่องจากอาจใช้เวลาเป็นหลายเดือนถึงหลายปีกว่าอาการต่างๆ จะหายไปหลังจากหยุดยา.
-minocycline-induced cutaneous hyperpigmentation มีอยู่ 4 ชนิด คือ
Type I เกิดขึ้นที่หน้าภายในเนื้อเยื่อที่อักเสบ,
type II เกิดขึ้นที่แขนและขา เป็น circumscribed distribution
type III เกิดเป็นสีน้ำตาลเหมือนดินเหนียวกระจายทั่วไปที่ผิวหนังส่วนที่โดนแสง, และ
type IV เกิดขึ้นที่ลำตัวภายใน scar tissue.

ในผู้ป่วยรายนี้ ไม่มีการเปลี่ยนแปลงของผื่นหลังจากกิน cefadroxil ไป 2 เดือน และผู้ป่วยขอกลับมากิน minocycline ต่อ.
