A 36-YOM had bleeding gums & LUQ abdominal pain for 6 days




   ชายอายุ 36 ปี มีเลือดออกจากเหงือกและปวดที่ท้องส่วนบนข้างซ้ายมา 6 วัน. ผู้ป่วยมีไข้ อ่อนเพลีย เบื่ออาหาร และน้ำหนักลด 4.5 กก. ใน 1 เดือน.
On physical examination, red, swollen gingivae (Panel A), tender submandibular lymph nodes, and a palpable spleen were noted.

    อายุน้อย ไข้ + น้ำหนักลดพอควร + เหงือกแบบนี้
คิดว่าเป้น gum hypertrophy with bleeding จาก DIC + splenomegaly ทำให้ plt ต่ำลง
เนื่องจากหัวข้อเป็น hemato : Acute leukemia เช่น AML M3 or M4 ก็สามารถให้อาการแบบนี้ได้ครับ
การรักษาในภาวะรีบด่วนคงต้องดู CBC ถ้ามีภาวะ hyperleukocytosis ก็ต้องรีบรักษาด้วยการทำ leukapheresis เพื่อป้องการอุดตันเส้นเลือดจากภาวะ hyperviscosity และ hydration เยอะ เพราะมีโอกาสเกิด tumor lysis syndrome ได้
ส่วนการจำเพาะ ถ้า acute promyelocytic leukemia ก็ให้ all tran retinoic acid (Vesanoid เห็น อ.พงเทพษ์ใช้อยู่นะคับ) +idarubicin
เอ่อถ้าไม่ใช่ก็ขอโทษด้วยนะคับ

   คุณหมอ nimbus ตอบถูกแล้วค่ะ คุณหมอเก่ง hematology นะคะ

Laboratory evaluation: a peripheral-blood white-cell count of 194,100 per cubic millimeter, with 44% blasts, and a peripheral-blood platelet count of 12,000 per cubic millimeter.
Examination of a bone marrow–biopsy specimen showed acute myelomonocytic leukemia with dysplastic eosinophils with a deletion of chromosome 16q and trisomy 22, karyotypic abnormalities (variant M4E).

Leukemic infiltration of the gingivae สัมพันธ์กับ monocytic variants ของ acute myelogenous leukemia. หลัง emergency treatment ด้วย plasmapheresis และ induction chemotherapy ด้วย cytarabine และ doxorubicin, the gingival infiltration หายไป (รูป B; รูปนี้ถ่ายที่ 3 สัปดาห์หลังรูป A). ต่อมา ผู้ป่วยได้รับ consolidation chemotherapy 3 cycles จนครบ และยังมี remission ที่อีก 1 ปีต่อมา.
