An 83-YOM had fallen down in his home

ชายอายุ 83 ปีหกล้มในบ้านของเขาเอง แรกรับผู้ป่วยสับสนและกระวนกระวาย
Physical examination revealed a swelling with bruising over his right sternoclavicular joint (รูป A) and periorbital bruising.
CBC: white-cell count 14x103/cu mm, C-reactive protein level 56 mg/L per liter, and urinalysis that revealed pyuria.
ได้ซัก social history และพบว่าผู้ป่วยไม่ได้ถูกทำร้าย

ขอลองตอบนะคะ อาจารย์
ปัญหา
1. ชายอายุ 83 ปีหกล้ม
2.ผู้ป่วยสับสนและกระวนกระวาย
3. swelling with bruising over his right sternoclavicular joint and periorbital bruising.
4. CBC: white-cell count 14000/cu mm, C-reactive protein level 56 mg/L per liter,
5. urinalysis that revealed pyuria.
ก็มี 2 ปัญหาใหญ่คือ
1. หกล้ม--> anterior dislocation of right sternoclavicular joint และperiorbital bruising. จาก CXR ไม่พบความผิดปกติอื่นๆ
2. leukocytosis, elevation of C-reactive protein level น่าจะเกิดจาก UTI ซึ่งอธิบายเรื่อง alteration of consciousness ของผป.ได้ถ้ามี urosepsis และเพราะผป.สูงอายุ

เรื่อง anterior dislocation of right sternoclavicular joint ให้ symptomatic treatment ไปก่อน
เรื่อง urosepsis เร่งด่วนกว่า ทำ UA, เอาปัสสาวะมาย้อมสีแกรม และส่งเพาะเชื้อ ให้ antibiotic (เลือก ceftriaxone ถ้าย้อมพบ Gram-negative rod) และ Iv fluid ค่ะ

CXR showed a right sternoclavicular dislocation, ซึ่งจากลักษณะทางคลินิกยืนยันว่าเป็นanterior dislocation.
Anterior sternoclavicular dislocation มักเป็นผลจาก indirect force ต่อไหล่ที่ทำให้ไหล่หมุนไปข้างหลัง แพทย์ต้องนึกถึงเสมอว่ามีโอกาสที่ abuse ทำให้เกิด injury แบบนี้
ผู้ป่วยได้รับการรักษา urinary tract infection ส่วน anterior sternoclavicular dislocation ได้รับการรักษาแบบ conservative ผู้ป่วยหายสับสน และหลังจากการทำกายภาพบำบัดเป็นระยะเวลาสั้นๆ เขาสามารถใช้แขนได้เหมือนเดิม.
