A 68-YOM with ascites & umbilical hernia had fluid came out from umbilicus

ชายอายุ 68 ปีซึ่งมี alcoholic cirrhosis, portal hypertension, ascites และ umbilical hernia อยู่เดิมมาที่ ER หลังจากไออย่างรุนแรงแล้วมีน้ำและไขมันไหลออกมาทางสะดือ ผู้ป่วยไม่มีอาการปวดท้องเลย
ก่อนหน้านี้ผู้ป่วยมี tense ascites แต่ไม่มีอาการอื่นๆ ไม่มีประวัติการผ่าตัดมาก่อน
Physical examination revealed scleral icterus, spider angiomata, a distended abdomen with a fluid wave, and a 4-cm segment of omentum protruding from the umbilicus and draining ascitic fluid.

1. Perforated umbilical hernia มีprecipitaing causeใดบ้างที่ทำให้เกิด rupture
2. ภาวะแทรกซ้อนของ ruptured umbilical hernia มีอะไรบ้าง
3. จะ manage อย่างไร

ขอตอบค่ะ
precipitating cause ของ การแตกของ umbilical hernia น่าจะเกิดจากการมี injury ต่อ umbilical hernia โดยตรงหรือภาวะที่ intraabdominal pressure สูงขึ้นเช่นจากการไอ อาเจียน
ส่วนภาวะแทรกซ้อนของการแตก คือ hypovolemia, การติดเชื้อ (peritonitis) ส่วนลำไส้ที่ไหลออกมาหรือติดอยู่ที่รูอาจเกิด strangulation ได้ค่ะ
Management
1. ให้ IV fluid และ prophylactic antibiotic
2. ปรึกษาศัลยกรรมค่ะ เพื่อจัดการกับ omentum ดูลำไส้ และเย็บปิด ซ่อมแซม umbilical hernia

In the operating room, strangulated omentum was found and resected, and the umbilical defect was repaired.

Umbilical hernia พบได้บ่อยในผู้ป่วยที่มี ascites และเป็นผลจากความดันในช่องท้องเพิ่มขึ้นร่วมกับมี defects ที่ผนังหน้าท้อง umbilical hernia อาจแตกออกมาในผู้ป่วยบางรายแต่พบได้น้อยมาก ปัจจัยที่กระตุ้นให้ umbilical hernia แตกได้แก่ local trauma, coughing, vomiting, หรือ esophagoscopy ภาวะ แทรกซ้อนของการแตกนี้คือ evisceration, confinement, หรือ strangulation of bowel, และ hypotension ที่เกิดจาก fluid shifts และ peritonitis.
