A 25-YOW had recurrent abdominal pain that first developed 10 yrs earlier




   หญิงอายุ 25 ปี ปวดท้องเป็นๆหายๆมานาน 10 ปี
Physical examination revealed mild tenderness in the left upper quadrant, without rebound or guarding.
Laboratory tests showed no significant abnormalities.

1. ในรูป A คือ coronal computed tomography (CT) ของ colon พบความผิดปกติอะไร
2. การวินิจฉัยคืออะไร

ใน CT scan พบมี gas อยู่ในผนังลำไส้ค่ะ
การวินิจฉัย: pneumatosis intestinalis

ผู้ป่วยมีอาการปวดท้องแต่ไม่รุนแรง เป็นมานาน 10 ปี ตรวจร่างกายไม่มี sign ของ bowel perforation หรือ peritonitis น่าจะ conservative treatment ได้และ investigate หาสาเหตุ

Coronal computed tomography (CT) (Panel A) and colonoscopy (Panel B, transverse colon) showed submucosal cysts confined to the splenic flexture and the distal transverse colon (arrows in Panels A and B), which were consistent with pneumatosis cystoides coli.

1 ปีก่อนมาตรวจคราวนี้ ผู้ป่วยปวดท้องแบบฉับพลันที่ left upper quadrant ได้ทำ CT พบว่ามีลมบริมาณน้อยอยู่นอกลำไส้ใหญ่ร่วมกับมีการอักเสบ และแพทย์ให้การวินิจฉัยว่ามี microperforation. ผู้ป่วยได้รับการรักษาโดยไม่ได้ผ่าตัด และได้ผลดี

   จากการปวดคงที่อยู่เฉพาะที่ และเป็นเฉพาะที่ส่วนหนึ่งของลำไส้ใหญ่ ผู้ป่วยจึงได้รับการทำ laparoscopic resection of the transverse colon and splenic flexure โดยไม่มีภาวะแทรกซ้อนใดๆ และผู้ป่วยหายปวดท้องหลังผ่าตัด

The pathological specimen showed the characteristic features of pneumatosis cystoides coli, air-filled cysts of various sizes in the submucosa and subserosa of the colon (รูป C, ลูกศร).

Pneumatosis cystoides coli สามารถเป็นได้ทั้งแบบ focal หรือ diffuse และเกิดขึ้นกับผู้ใหญ่เป็นสำคัญ ภาวะนี้มักไม่ทำให้เกิดอาการแต่ก็สามารถทำให้เกิดลำไส้อุดตันหรือ pneumoperitoneum และปวดท้องเป็นครั้งคราวหรือปวดตลอดเวลาก็ได้
