A 77-YOM with DM had fever & left-sided neck + shoulder pain for 4 days

ชายอายุ 77 ปีซึ่งเป็นเบาหวานอยู่เดิม มีไข้และปวดคอด้านซ้ายและไหล่ซ้ายมากขึ้นมา 4 วัน ร่วมกับมีอาการชาและรู้สึกแปล๊บๆ ที่นิ้วมือซ้าย
Physical examination revealed severely restricted neck movement and marked palpable neck tenderness over the fourth, fifth, sixth, and seventh cervical vertebrae.

ผู้ป่วยได้รับการตรวจ integrated positron-emission tomographic and
computed tomographic scanning (เนื่องจากเขามี pacemaker อยู่และไม่สามารถทำ MRI ได้) ดังในรูป A และ B

1. การวินิจฉัยคืออะไร
2. จะ manage อย่างไรต่อไป

ขอตอบค่ะ
1. acute vertebral osteomyelitis ค่ะ เชื้อก่อโรคน่าจะเป็น Staphylococcus aureus, Streptococcus spp., หรือ GNR เช่น E. coli
2. ทำ blood culture, UA และ urine culture แล้วเริ่มให้ empirical antibiotic ดิฉันจะให้ ceftriaxone 2 g iv OD
3. ปรึกษา orthopedics เผื่อว่าจะได้ pus มาย้อม Gram stain+culture ค่ะ

จาก integrated positron-emission tomographic and computed tomographic scanning พบว่ามี: an intense focal area of abnormal hypermetabolism confined to the left aspect of the fourth cervical vertebral body (ลูกศร รูป A) that was consistent with the presence of an infection.

การเพาะเชื้อของเลือดขึ้น Enterococcus faecalis ซึ่งไวต่อ ampicillin.

คุณหมอจะเลือกให้ยาปฏิชีวนะอะไรต่อ และจะรักษานานเท่าใดคะ

คำถาม จะเลือกให้ยาปฏิชีวนะอะไรต่อ และจะรักษานานเท่าใด
ตอบ เลือกใช้ ampicillin+gentamicin ค่ะ ให้ยานาน 6 สัปดาห์ค่ะ
