A 48-YOM with peptic ulcer had sudden onset intense epigastric pain for 4 hrs

ชายอายุ 48 ปีซึ่งได้รับการรักษา peptic ulcer disease ด้วยยากินอยู่ มีอาการปวดท้องที่ลิ้นปี่อย่างรุนแรงขึ้นมาทันทีโดยไม่มีอาเจียน 4 ชั่วโมงก่อนมารพ.
On examination, he had rigidity of the abdominal muscles in the epigastrium and tympanicity on percussion in the right upper quadrant.

คิดว่าผู้ป่วยมี peptic perforation ค่ะ การวินิจฉัยแยกโรคคือ acute pancreatitis ซึ่งเกิดขึ้นได้จาก penetration of peptic ulcer เข้า pancreas ค่ะ

management
ช่วงแรก
- NPO, ให้ IV fluid
-ส่งฟิล์ม acute abdomen series ก่อนค่ะ ดูว่ามี free air ใต้ dome of diaphragm หรือไม่
-ปรึกษาศัลยกรรมค่ะ

รูป A: ภาพรังสีทรวงอกของผู้ป่วย พบ bilateral pneumoperitoneum (ลูกศร).

Exploratory laparotomy: a perforated duodenal ulcer with callous borders (รูป B).
แพทย์ได้เย็บปิดแผล 3 stitch และใช้ omental patch
ผู้ป่วยไม่มีภาวะแทรกซ้อนหลังผ่าตัดใดๆ และไม่มี recurrence.
