A 76-YOW had abnormal CXR




   หญิงอายุ 76 ปีได้รับการดูแลรักษาใน ICU เพื่อ hemodynamic monitoring หลังการผ่าตัด vulvectomy and inguinal lymph-node dissection for vulvar carcinoma. ผู้ป่วยมีประวัติ (1) deep venous thrombosis, (2) type 2 diabetes mellitus, และ (3) atrial fibrillation, with a slightly reduced tolerance for exercise (New York Heart Association functional class II) and peripheral edema.
-ไม่มีประวัติ mitral-valve disease และตรวจร่างกายไม่พบ murmur
-ผู้ป่วยเคยได้รับการผ่าตัดต้อกระจกและ drain subdural hematoma โดยไม่มีภาวะแทรกซ้อนใดๆ

1. ในรูปคือ chest radiographs (Panels A and B) พบความผิดปกติอะไร
2. การวินิจฉัยคืออะไร

   ขอตอบค่ะ
1. ผุ้ป่วยมี calcification ของ pericardium ทั่วไป เห็นทั้งใน PA และ lateral view
2. pericardial calcification มักเกิดตามหลังการมี pericarditis หรือ trauma.
pericarditis ที่เกิดจากการติดเชื้อ ได้แก่ virus (เช่น coxsackievirus, influenza A, influenza B), tuberculosis และ histoplasmosis โรคของ pericardium ที่เกิดจาก SLE, rheumatic heart disease, uremic pericarditis, และ hemopericardium (posttrauma หรือ cardiac surgery) อาจทำให้เกิด pericardial calcification ได้ pericardial tumors, เช่น intrapericardial teratomas และ pericardial cysts ก็อาจมี calcification ได้

ผู้ป่วยที่มี pericardial calcification มีอยู่ 50-70% ที่มี constrictive pericarditis. ในกรณีที่มี extensive calcification ผู้ป่วยอาจมีหรือไม่มี pericardial constriction ก็ได้

   จาก chest radiographs (รูป A และ B) และ computed tomographic scan (รูป C) พบว่ามี extensive pericardial calcification. ผู้ป่วยไม่มีประวัติของการเป็นวัณโรค แต่ตอนเป็นเด็ก เธอเคยมี Sydenham”s chorea (ซึ่งมีชื่ออื่นคือ St. Vitus”s dance หรือ chorea minor), Sydenham”s chorea เป็น “ a self-limited, autoimmune-mediated, choreiform movement disorder that follows infection with Streptococcus pyogenes”
ในรายนี้ แพทย์คิดว่า pericardial calcification เป็นผลจากการมี rheumatic fever ตั้งแต่วัยเด็ก
