A 58 yrs male with 2 episode of focal seizurecase ผู้ชายอายุ 58 ปี underlying COPD มาพบแพทย์ด้วยอาการ 30 นาที PTA มีอาการเกร็งกระตุกขาขวาข้างเดียว เป็นอยู่ประมาณ 1 นาที อาการหายไปเอง ไม่รู้สึกตัวดีตลอด ปวดศีรษะ ไม่มีตาพร่าหรือเห็นภาพซ้อน ไม่มีคลื่นไส้อาเจียน ระหว่างพบแพทย์ที่ OPD มีอาการขาขวากระตุกอีก 2 ครั้ง ครั้งละประมาณ 1 นาที อาการหายไปเองเช่นกัน PH: เคยมีอาการเช่นนี้มาแล้ว 1 ครั้งเมื่อประมาณ 4 เดือนที่ผ่านมาโดยครั้งนั้นมีขาขวากระตุกตามด้วยแขนขวากระตุกร่วมด้วย มา รพ.ได้ adimit observe อาการชัก ไม่พบว่ามีชักกระตุกอีก จนถึงครั้งนี้ Physical exam: BP 150/80 mm.Hg., P 83/min, R 20/min Neuro sign: good conscious, E4V5M6, pupil 3 mm BRTL, no facial palsy Motor Gr. V all, DTR 2+, BBK: platar response, Clonus positive Lab: CBC, electrolyte: WNL CT-Brain: The study reveals several small cystic lesions with internal tiny hyperdense dots at bioccipital, bifrontal, bitemporal and Lt.parietal lobes, varying about 0.8-2.4 cm in size. Faint hyperdense lesions at Lt.frontal, Lt.temporal, Rt.parietal and Lt.occipital could be faint calcifications rather than hemorrhagic spots. There is a 1-cm ring enhancing lesion with prilesional edema at Lt. parietal lobe. No midline shift or brain herniation is seen. The visualized orbits, PNS and mastoid air cells are unremarkable. Imp: Findings as mentioned, neurocysticercosis is considered. คำถาม 1. ในผู้ป่วยรายนี้การให้ antiparasitic drug มี risk และ benefit อย่างไรบ้าง 2. เมื่อเทียบ risk และ benefit แล้ว ควรให้การรักษาด้วย antiparasitic drug หรือไม่ และ ควรเลือกยาตัวใดระหว่าง praziquantel และ albendazole 3. ในรายที่จำเป็นต้องได้รับการรักษาด้วย antiparasitic drugs จำเป็นต้องได้ dexamethasone ร่วมด้วยในกรณีใดบ้าง 4. หลังการรักษาด้วย antiparasitic drug จำเป็นต้องให้ยากันชักต่อไปอีกหรือไม่ Posted by : Ethambutol , E-mail : (liv_grace@hotmail.com) , Date : 2009-12-27 , Time : 08:49:44 , From IP : 117.47.181.239 |
