A 62-YOM had digital gangrene affecting the right hand for 1 mo.




   ชายอายุ 62 ปีมาด้วย digital gangrene ของนิ้วมือขวามานาน 1 เดือน (รูป A). นิ้ว 1 นิ้วที่มือขวาถูกตัดไปเมื่อ 20 ปีก่อนหลังอุบัติเหตุ
ผิวหนังที่แขนและขามีลักษณะดังในรูป B (reddish-blue, mottled discolorations in a net-like pattern) และมี palpable cutaneous nodules. การตรวจร่างกายอื่นๆอยู่ในเกณฑ์ปกติ
ผู้ป่วยไม่สูบบุหรี่
CBC, urinalysis, renal function และ liver function อยู่ในเกณฑ์ปกติ
Tests for antineutrophil cytoplasmic autoantibody, hepatitis B virus, and hepatitis C virus ได้ผลลบทั้งหมด
ไม่พบความผิดปกติของ computed tomography ของ head, neck, และ thorax echocardiogram: ปกติ

ชายอายุ 62 ปี มีปัญหาคือ
1. digital gangrene ของนิ้วมือขวามานาน 1 เดือน
2. ผิวหนังที่แขนและขา reddish-blue, mottled discolorations in a net-like pattern
3. มี palpable cutaneous nodules.
4. biopsy specimen ของ cutaneous nodule: ลักษณะเป็นหลอดเลือดที่อุดตันและมี cellular infiltration รอบๆ ทั่วๆไป = arteritis

การวินิจฉัย: polyarteritis nodosa ค่ะ
การรักษา: corticosteroid

การวินิจฉัยคือ cutaneous polyarteritis nodosa
positive findings ซึ่งเป็น clue สำหรับการวินิจฉัย polyarteritis nodosa ในผป.รายนี้ คือ
1. digital gangrene in the right hand (รูป A).
2. skin of his arms and legs had mottled discolorations with palpable cutaneous nodules (รูป B) ซึ่งมีชื่อเรียกว่า livedo reticularis
4. A deep incisional-biopsy specimen from a cutaneous nodule revealed occluded middle-sized arteries with lymphoplasmacellular and neutrophilic infiltration (รูป C)

ผู้ป่วยได้รับการรักษาด้วยการตัดนิ้วส่วนที่มี necrosis ออกไป และได้รับ glucocorticoid treatment ทั้งๆที่ได้รับ glucocorticoid อยู่ ก็ยังมี cutaneous nodules ใหม่เกิดขึ้น. การรักษาด้วย cyclophosphamide ทำให้ควบคุมโรคได้
1 ปีต่อมา ผู้ป่วยยังมี complete remission และ immunosuppressive therapy ถูกลดขนาดลงทีละน้อย
